Made a mistake :/

Sorry about this, can't PM an admin or else I would do.
I've registered my 980ti on here with the wrong purchase date because I looked at the wrong receipt.  I've since uploaded the correct receipt but can't change the purchase date for my item.  It has died on me a few days ago and I need to sort out warranty.
Being in the UK, where will I be needing to send it? Back to the supplier (Scan) or back to yourselves in America I'm assuming?

    Well according to the Royal Mail tracking service, it's in their hands now...amused me no end to find the RMA depot being down the road from me after my fears of international postage lol

    I've had the card replaced under warranty, thanks EVGA for that... but just over a week of putting the new card in I've got the same fault again.
    Already replaced the motherboard and powersupply so now I'm at a loss, no idea what's caused the second one to be faulty.
    What do I do?

    No, you see my main concern is if something has caused it to be faulty.  Replacing it would be pointless if something is going to break it again :/

    Did you replace the motherboard & psu before the second card failed or after the second card failed?

    After the first card had failed, everything initially pointed at the psu, so I bought a new one which didn't fix the problem...after that, everything pointed at the asus anti surge protection being ropey at best so I replaced that with new aswell.  Turned out it was the graphics card that was at fault so had that replaced.  
    Problem is now it's happened again, what is causing it and how do I stop it happening again? :/

    Hardware can fail at any time. I'm thinking you just received another faulty gpu. All you can do is rma it again and hope for the best. 
     
    If you're paranoid you could...
     
    #1 Rma your motherboard, psu & gpu.
    #2 Replace motherboard, psu & gpu with a new ones.
