Motherboard is GA-Z170X-Gaming 3 (rev. 1.1  with CPU i7-6700k
RAM is Adata xpg z1 using XMP profile 2800mhz (Have ran memtest multiple times. is fine)
CPU is at stock 1.2v and stock clocks
 
Only errors I get in event log is the nvidia driver crashed every time I game. Furthermore, I find it very strange that the card has "decayed" in stability so much since i got it 1 month ago. I ran uningine valley when i first got the card to find if i could up the core clocks any at 120% power target and was able to do about +50mhz without touching volts. Nothing in my configuration as changed and I can no longer even complete the valley benchmark at stock settings and 120% power target without nvidia driver quickly crashing. This is after a full system reformat too. When GPU parts are failing does stability typically decay like this?

Not looking good for the GPU.....
 
Slowly getting worse -- BIOS corruption issue (Sajin mentioned) ---- maybe a poor connection somewhere that is getting worse
 
Short of testing the GPU in another PC or another GPU this PC ..... (my kitchen sink list)
 
Your system should not be pulling even 500 watts, so PSU should not be hard pressed -- how old is it ?
 
Any large appliance on the same circuit as the PC ?  (laser printer, microwave, A/C, Fridge)
 
Win 10 ? - if yes turnoff the Xbox DVR (on by default and its a resource hog)
 
Your MB has sound and Nvidia drive will load sound drivers - Check Device Manager -- (only run 1 sound driver for stability)
 
Which Nvidia driver have you tested ? ( Try 378.92)
 
Run DDU in default settings safe mode, with reboot when changing drivers
 
When you Install Nvidia driver -- Use Custom install -- Only GPU driver & PhysX -- Clean install ---Only at this time
 
Your running EVGA Precision OCX ? version 6.1.14 ?
 Add 30 - 40 % voltage and test
If using OCX - turn Off  --- OSD
 
Confirm GPU/card is fully seated and locked in PCie slot -- test in other slots
 
Make sure power cables are fully seated and latched -- sometimes they get tight before that are in all the way.
 
You gaming online ?  if so turn Off hardware acceleration in your browser
 
How do you have Nvidia Control Panel setup ?

Cool GTX thank you for the help. I tried in other PCI-E slot, same result. I only have driver + physx installed no other component (clean install checked). I re-did the cables to the PSU twice, using different set each time to confirm they were not the issue. tried +100% voltage, same crash. Checked every possible cable connection to motherboard and PSU all are tight and re-snapped to confirm. OSD never used. At this point I have given up and it seems to be a faulty card because of the fact that the issue has gotten worse and worse by the day at the moment. It is sometimes crashing now in non-3d in windows with artifacts. No other heavy duty device on circuit as I am on 2nd story and no appliances on this floor. Kill-a-Watt meter is reporting that my voltage is fine from wall.
 
Prior to this I used gtx 1080 FTW2 ICX with no issues same system config.

Interesting to note, everytime EVGA precision XOC tries to load on startup, i get crazy artifacts and crash on windows desktop alone so I cant even start PC properly without solely relying on Igfx. Now that I think of it, this problem also started when I first tried evga precision xoc and wanted to up the power target to 120%. I think evga precision is killing our cards possibly even with no profiles saved. Another user is also having same issue.
 
Upon uninstalling precision xoc I am finally able to log into windows but I fear it might have killed my card when i first tried using it.

I highly doubt precision damaged your card. Time to send the card in.

is it a thermal issue? i would contact support - theyre pretty good about letting you reseat the heatsink without voiding warranty.  They let me do that during the whole 1080 iCX fiasco last year
