Has anyone ran into this issue before? Using windows 10 fully updated. I've tried both headers on the motherboard and neither work. I have no missing drivers under the device manager. USB 2.0 header works just fine.

Do a support ticket with EVGA. I have this board and I'm using both 3.0 headers plug and play with no issue. Sounds like an issue with your board.

Do you think it's an issue with the case or no?

You could look in the BIOS to verify the USB 3.0 headers are enabled.  By default I believe they should be, but no harm verifying...

The board comes with a 3.0 header bracket in the box. I would try that to test.
Could check your bios but should be enabled by default. I never had to enable this on my board. Just always worked.

No it's definitely enabled in the BIOS. There wouldn't be any software issues in Win 10? There's nothing in device manager that needs to be updated in regards to USB stuff.

When you look in the Device Manager > Universal Serial Bus Controllers > do you see the Intel USB 3.0 eXtensible Host Controller?  And also the USB Root Hub (USB 3.0)?

Do the USB 3.0 ports work in the I/O panel on the back or your case?  If so, it could be a bad cable connecting from the motherboards header to the front USB 3.0 ports.  Might want to check those connections on both ends (header & front 3.0 connector) to make sure they're seated firmly.  Also the suggestion by cjsquirrel of plugging the USB 3.0 bracket into the header on the board to test should rule out or pinpoint cable or header issue's.

There is also a driver on EVGA's site for USB 3.1 for your board running W10 64bit.  Not sure it's relevant here though > http://cdn.evga.com/Driver/Z270/USB31.zip
