Hey all as I saw that my avatar just vanished, I wanted to re-upload it.
The following method kind of worked as I managed to trick the bug in some way, so here my work around to help those that have this as well:
 
After trying to upload your new avatar via the old method this happens:

 
So then I try again and I get this, so I do Crop:

 
The I go to Crop, but don't touch anything, just press OK & Save it:

 
And presto that worked:

 
The croppng doesn't help because that gave a Zoomed in result, it's super rare you even get  the chance top get the ability to crop it.
 
But every time I keep retrying my old method this keeps happening every time I want to setup my avatar

 
Also deleting my history has no effect I use the latest version of Mozilla Waterfox  with Windows7Pro x64 UK + SP1

Hmm it works now, thanks for the one for fixing it, unless it was random.
That is something I can't see from my end.

Me thinks its a random bug, modrigs does the same.

And again my avatar is gone, I never removed it it seems it just disappears... dunno what is going on tbh...

Happens to me especially when I try using a .gif image.

Hmm I am just using jpegs, strange though,  worst thing it's random too.
I always resized them to 640x 480 and let the site compress them so image quality is better.
That to which should not break em, been doing it like this since 2009, even tried to resize em to 200 x 200px that too didn't do anything.
 
If I could find out what breaks it. only then a workaround could be made or better a fix.

Well it works now, I dunno what causes it still would be interested since if we know what triggers it then a workaround or even better a fix can be made to prevent it.
