EVGA GeForce GTX 1060 - Display Driver Has Stopped Responding And Has Recovered

Have researched all other "same error" issues within the forum, but all seem to be unique respectively.
 
The card works fine 98% of the time, its only on the rare occasion that I am watching a video of one sort or another, when
the entire screen just goes to black.
 
I then, "have to" get up, and unplug the HDMI cable out of the 1060, and plug it back in.
 
Then the screen comes back on again, but, everything, ...and I do mean EVERYTHING, is slammed up in the top left hand
corner of my screen. Icons, any browsers that were open, etc. And I am left with the error message that I have attached.
 

 
This card, 1060, is the reason I downloaded and had to install the nifty little utility entitled, "DesktopOK". DesktopOK puts
my entire desktop back to where I had everything every time this happens. Don't know what I'd do with out DesktopOK.
 
(...as a side note, unrelated to this issue, every single time I ever install the newest drivers for the 1060, the driver installation totally jacks up the order of all my Desktop icons, every time. Don't thing this "should" have to happen).
 
So I'll try to give as much info as I can think to type.
 
This has been happening ever since I got the 1060, never had it happen with any of my other EVGA's.
 
Every time I install the latest driver, I always go into Custom, and do a clean install.
 
The problem doesn't do anything fatal, like a complete BSOD reboot, or crash, just kills the video until I unplug and re-plug
the HDMI cable from the 1060 again.
 
All settings on the 1060 are stock, I'm not a modder.
 
Normal, nominal GPU temp for me, 34C.

OS:Windows 7 Ultimate x64
 
MB:Asus P5K Deluxe Chipset Intel P35/AMI BIOS v8.12/8GB System Mem
 
CPU:Intel Core2 X9650 Extreme CPU
 
Monitor: Samsung UN43KU7000 /Resolution I Run At: 3840x2160 4k @ 60Hz @ 4:4:4 (Connection from 1060 to monitor is via HDMI cable).

    There are a few things you can try:
     
    1. Different port on the monitor
    2. Different HDMI cable
    3. Try using the DVI or D-Port
