EVGA 1080 Hybrid Thermal Throttle without being at Max MHz??

My hybrid has sometimes (not all the time) been thermal throttling itself while being on max load and max fan speeds with temps pinned at 95C. I dont' understand the issue. I'm getting horrible FPS while the MHz stays below 500. The fan is running, there is ariflow, i can feel the water running through the pipes but still burning hot? Any ideas? Pic related
The weird thing is that is doesn't always happen.
 
Additional Info: Driver version 397.64 released on 05/09/2018
Card was bought new around 8 months ago
No visible dust on card and nothing out of the ordinary
Doesn't always happen, maybe once every 2 weeks
Other components:
i5-4690k
gigabyte z97x-SLI-CF
 
The picture shows clock speed at 342MHz and a temp at 94C with GPU fan running at 100%

    Your pump is not running with temps that high. If you're running a bunch of other apps that monitor sensors it might cause an issue. Otherwise it is probably the pump and you'll need to RMA the GPU.

    That's odd i thought that i felt water running through it but is there a sensor to determine if the pump is functioning or not?

    There's no pump sensor. It's a 12v pump that runs at one speed. It's pretty simple. If the water is not flowing, the GPU temp is going to skyrocket and the GPU will throttle the clocks due to temps.  You could double check all the connections on the GPU. What is the orientation of the radiator in relation to the GPU? Air bubble?
