980ti video card removed errors

First and foremost let me say this is the third 980ti that I have had issues with, the first two were RMA's. This one is now out of warranty and I am constantly getting the attached error from windows. This is only happening when I am playing Andromeda Mass Effect 3. I do not have the issue with other games. My bios is current and have the latest Nvidia drivers 417.71.
 
My system specs are:
I7 4790k no O/C
EVGA 980ti SC no O/C
Asus Maximus Hero 7 MB
GSkill Trident X 2133 Mhz
Corsair RM1000i Power Supply
Windows 10 64bit
 
I have removed, cleaned the video card and contacts. I thought I had found the problem, being a faulty HDMI cable. I replaced it with a Display Port Cable and the problem still persists, although not as often. It gets very frustrating when I am in the final wave of a game and the card disconnects. I have been using EVGA cards for over 15 years. This is the only card that I have ever had issues with, bad card series?

    You are not alone steel700 there are many reports of the same error.  Good news is ... it's not your card , it's the game

    See if doing #4 listed here helps get rid of the issue.

    It did not help. I pulled a GTX 780 from a system a gave to my grandson. I have had no issues with the game crashing, have been playing non-stop for 8 hours. I put the 980 ti back in just a moment ago, it crashed with the same error. Definitly EVGA refurb that was sent to me thanks for the assist, I appreciate the feedback.

    It looks like you are using driver version 417.71. I would recommend using an NVIDIA driver starting with a 3, if possible. The 4xx.xx and higher drivers tend to not work so well for older video cards. I don't remember the specific driver I am using, but I believe it is something like 397.xx or something. I have been using it on my two laptops and on my PC; all with 10xx series NVIDIA video cards.
