Instead of staying at 139mhz it randomly jumps to 605 and 1569 and stays there for a couple minutes before going back down to 139. Any idea what could be causing this?

A process running in the background is the most likely cause.

In task manager I am at 0% GPU with chrome closed. I only see client server run time process that jumps around from 0-4%. Even at 0% usage it still takes a while to go back down to 139mhz from 605

Perform a clean boot of windows: - Instructions on how to perform a clean boot for windows vista, 7, 8/8.1 & 10 can be found here
Do that and let me know if you continue to have issues.

Just did the clean boot and restarted but problem still persists

Ok... Download process explorer and suspend searchui.exe. See if that fixes the issue for you.

If suspending searchui.exe doesn't fix the issue then I would recommend that you scan your system with mbam.

Suspending searchui didnt help and I just scanned it with malwarebytes before making this topic. It only found local files with chrome which is apparently a known bug.

I just turned off any active tiles and ms onedrive and xbox dvr. I'm about to go to bed but I'll chekc out drive indexing in the morning.

Inside process explorer select view (top left), select columns, select process gpu, check mark gpu usage. That will create a column on the main screen called "GPU", you will be able to monitor what process is using your GPU the most when idle. See if you notice anything with that.

Yes your title threw me off "1080ti ftw3 clock keeps boosting when just in desktop" It is completely normal as Sajin pointed out for clocks to bounce all over when browsing. If you'd had not specifically claimed it was while you were JUST on the Desktop, and not actually browsing, a post or two would have got you the right answer. Also the higher refresh and resolution you are running the higher your average GPU usage will be even while doing something as lightweight as browsing.
