I am having a problem getting past the 4.2 GHz mark on the CPU, when it blue screens, it mangles the video in the process and I have to reload those. Here is the template

Mother Board ( EVGA X58 Classified E-760-A1 )

Maximum Payload Size ( 4096 )

Power Management Setup
ACPI Suspend Type ( S3 )
HPET Support ( Enabled )
HPET Mode ( 64-bit )

My temps when running at 100% like this are as follows:
Core 1: 65 c
Core 2 : 54 c
Core 3 : 62 c
Core 4 : 52 c

running the fans between 1500 and 2000 rpm and water temp on the CPU loop is reaching about 29.2 C
Air cooling on the memory.
Crashes happen when running Prime 95 with the last blue screen with a 000000000a error code. I have been getting 00000101a and a few 000000055a errors as well. Have not seen the 125 error yet. I have manually pushed a few of the clocks and that is how I got to 4.2 but if I load it down for any length of time I will crash or freeze up.

Getting temps of 68, 61, 68 and 58 with intelburn utility.
Ran the tests at very high, did not have enough memory for maximum but oh well. The test says stable at this level.

I don't know why really but I think you'll get more help in the EVGA X58 SLI Forum instead. This thread  under normal circumstances would be the right place for normal logically thinking adults but they are a bunch of cranky geekinoids up there on there shiny hill. Tread lightly and head up there if you want some help with this. They might slap you around and laugh at you a bit  in the end I think it will be worth your effort and you can always come back here with us low life if you don't fit in. LOL
    Good Luck

edit:removed the unnecessary sarcasm so as to get to the point

So here are some thoughts on some points that maybe you havent gone through.  Please don't take these settings as gospel because as you know every chip is diff but heres what I got (to the right of your specs 4.5 ghz)........

    Frequency Control
    CPU Clock Ratio ( 31X )      20
    CPU Host Frequency (Mhz) ( 136 )     215
    MCH Strap ( Auto )
CPU Uncore Frequency (Mhz) (AUTO )    2X whatever your ram setting is...in your case 24-25
    CPU Clock Skew ( 0 ps )
    Spread Spectrum ( Disabled )
    PCIE Frequency (Mhz) ( 100 )

    Memory Feature
    Memory Speed ( Standard )
Memory Control Setting ( Enabled )
Memory Frequency ( 1600Mhz / 2:12 )       I could not find your ram but as long as you are NOT crossing over the specs on the ram with the oc, then you should be okay.
    Channel Interleave Setting ( 6 Way )
    Rank Interleave Setting ( 4 Way )
Memory Low Gap ( AUTO )
    tCL Setting ( 7 )
    tRCD Setting ( 8 )
    tRP Setting ( 7 )
    tRAS Setting ( 24 )
    tRFC Setting ( 88 )
    Command Rate ( 1t )

    Voltage Control
EVGA VDroop Control ( With VDroop )    without vdroop
    CPU VCore ( 1.3625 )          1.375
    CPU VTT Voltage ( +400 )    auto
    CPU PLL VCore ( 1.905 )      auto
IOH PLL VCore ( 1.815)        auto
    DIMM Voltage ( 1.7 )            1.65
    DIMM DQ Vref ( +0mV )       auto
    QPI PLL VCore ( 1.275 )       1.35
    IOH VCore ( Auto )               1.33866
    IOH/ICH I/O Voltage ( Auto )
VTT PWM Frequency ( 610 KHZ )   does not need to be this high but you can mess with it
CPU PWM Frequency ( 1210 KHZ ) does not need to be this high but you can mess with it
    CPU Impedance ( AUTO )
QPI Signal Compensation ( AUTO )
    ICH VCore ( Auto )  1.33866

    CPU Feature
    Intel SpeedStep ( Disabled )
Turbo Mode Function ( Enabled )
    CxE Function ( Disabled )
Execute Disable Bit ( Disabled )
    Virtualization Technology ( Enabled )
Intel HT Technology ( Enabled )
Active Processor Cores ( All )
QPI Control Settings ( Enabled )
QPI Link Fast Mode ( Enabled )
QPI Frequency Selection ( 6.400 GT/s )  4.6 but you can try both because I know that chip was made for 6.4gt/s
    OC Recovery ( Disabled )
    Good Luck,
