VMT.exe test results and questions(Video Memory Test)

First, the results
Application command line: vmt.exe +32 2048
I hit YES on all of these to go to 640x480x16 mode
512x512 ignore color bit mask = true, use onscreen buffer = false

I have run this since I have had some issues with my driver service restarting during GW2 and BF3 play.  Any thoughts on what could be happening?  What would be my best course of action?

    I highly suggest you not use such outdated programs, as they cannot produce any meaningful test results and might even be unwise to use....
     
    Driver failure is not related to video memory failure
    Memory failure produces on screen artifacts that are impossible to miss.
     
    Now drivers -can- fail if you improperly use settings that are to high for the amount of Vram you have available...
    Buts that unlikely with a GTX 680 
     
    Please post your complete system config specs, monitor config and the exact issues you are seeing...
     
    You most likely have a config, running background program or other issue that is causing your driver to fail.

    on GDDR5 enabled products, such memory tests won't actually report faults as GDDR5 spec requires ECC unless the issue exists within the memory controller itself.

    Sound:
    -Onboard 7.1 ASrock M3A770DE
    -Offboard Bulldog 7.1 PCI card
    -Reason: I like to have multiple voice chat programs open and not reverb back through on TS, Mumble, and Vent and do audio recording/sampling without disabling my sound from my main playback device: Onboard.
     
    3xHanns-G 24" monitors
    -Center:HK241DPB
    -Left/Right:HL249DPB
    -Resolution: 5760x1080 Surround
     
    Issue:  BF3 and GW2 crash the NVDrvSvc(Error 4101) when surround is enabled for 5760x1080x32x60Hz
    resolution.  All other applications do not when Surround is enabled. 
    BF3 and GW2 work flawlessly in single-monitor 1920x1080x32x60Hz mode.
    Furmark(45 min test), Precision Tuner X(2 hour test), OCCT(1.5 hour test), Farbrausch - Magellan(Demo scene demo) all work flawlessly as well at 5760x1080x32x60Hz
     
    Last 3 FINAL VERSION Drivers tried w/"clean install option" enabled and only allowing the driver and physx to be installed.
