The problem from what I can see in his/her description:  Under "Device Manger" "Universal Serial Bus controllers"  there is a VIA device that has the Message posted as the Issue i.e.:
    " VIA USB 3.0 Root Hub : Code 10 "
I don't know how to solve the problem, but I hope I have defined it correctly.

After you install correct driver IMO you should check to see if the old drivers were removed or just Hidden! See below and Read NOTEs at the bottom before you start.
To check and see if you have wrong drivers (FTW/SLI) from previous installs present on your hardware; you must set a flag for device manager from the cmd box.  In the Start Menu, click on All Programs and then Accessories.  Now right click on the Command Prompt and click on Run as administrator.    Now type in this  FLAG/CMD:  “set devmgr_show_nonpresent_devices=1” (no quotes)
Next type ”devmgmt.msc “  (as administrator)
Once you are in Device Manager, go to the "View" menu, select  “Show Hidden Devices”.  This will show all the device drivers including things that aren’t currently installed in your computer.
You can right-click on the driver and then choose Uninstall from the menu to remove the drivers for that old hardware.
This procedure can resolve a lot of weird issues, and even increase performance on some machines where you’ve upgraded drivers a ton of times.
NOTE: the grayed out drivers may OR may Not still be needed.  Before deleteing make sure they are not for some devices (like USBs or some parts of media player).

Please go into programs and features. If you have a NEC usb driver, or an Asmedia driver please uninstall them then install this file for P67 FTW: http://cdn.evga.com/driver/P67FTW_USB30.zip
    Or this for SLI: http://cdn.evga.com/bios/P67_USB30.zip

LVCoyote,  I and many others have been using this method for years.   It is slightly different from XP, but it works in x64 Vista and Windows 7 x64 prof.  I used it just last night.   Stupid q by me... are you sure you typed or even copied and Pasted (that's what I did last night) the "   set devmgr_show_nonpresent_devices=1  " command in Administrator mode of CMD.  It does not work from "Run"
Go to Start, find the Command Prompt, right click and select Run as Administrator .
Let me know (perhaps it does not work in the HOME versions).

I copy and pasted the entire line including FLAG/CMD, amd I not supposed to have that in there??  And yes I took out the quotes.  I'm running Win7 Pro

I am having this same exact problem and I am using the correct drivers, I even tried the lastest 1.50B drivers from VIA's website and it still does not work. All USB 3.0 ports are dead, plugging a flash drive into all four ports results in the flash drive not even being detected.

    What else could be wrong?

Strange, I solved my own problem in a wierd way.
I unplugged my second GTX 580, and unplugged my Soundblaster XiFi and booted into Windows. The exclamation point went away and a additional VIA USB 2.0 hub installed. I plugged my flash memeory stick into all ports and they were all working.

I put back the second GTX580 and everything was okay.

I put back the Soundblaster XiFi so that it was now the same as when I was having the problem and guess what, everything was STILL okay.

This is very wierd, but I am happy it's working now. Is there some screwed up thing going on that the drivers fail to install with all these cards plugged in, but once they are installed, you can reinstall your cards and all is okay?

Just posting this here as something else to try if you have problems and have more than a video card plugged into the slots.

Usually with any USB device issues in Device Manager, you can just delete the entry and do a rescan for new hardware and it will straighten itself out.

I have also used the show hidden devices thing for years. Not sure what he meant by flag/cmd unless he menat hit the windows key and open a command prompt.  I have mine saved as a batch file and just run it as an administrator.  Good for clearing out all those old volume shadow backup entreis that are no longer really there after a disk cleanup.

Yes, I did all that, followed this guide to the letter too, nothing fixed mine until I pulled all my cards cept one video, booted into Windows 7 once, and then reinstalled all my cards. Very strange.

Try disabling USB 3.0 in the BIOS, boot into Windows, then reboot and re-enable USB 3.0.

Hey everyone, new to the forums here. Built my computer with z68 ftw board a year or two back now. ViaHub3.sys caused blue screen and I had to do an entire fresh install of windows and once I got it up and running VIA Usb 3 Root Hub device was giving me code 10. Followed the advice above by disabling usb 3.0 in bios, boot to windows, restart, enable usb 3.0 and now it works fine no problems so we will see how long it holds up. Thanks joaks87! Goodluck!
