nvlddmkm stopped working (black screen) - gtx 970

Hello, since december I have 04G-p4-2974-kr. Only now I feel problems with more demanding games like witcher or Overwatch. Throws an error driver Nvlddmkm or Faulting application name: dwm.exe/dwmcore.dll. 
Located the error and it turned out that the card is working properly when i changed the power limit the value to 90%. Unfortunately, this setting is not fully move 144fps and this is quite frustrating. Following this step, it turned out that the gpu-z appears bios 84.04.36.00.70. Should I do an upgrade? I Perform windows format, I tested different drivers, changing the voltage on the card and changed clock below standards and this do not give results - there is not issue with temp. Also i changed registry tdr key, nothing helped. Sometimes furmark run much longer than Witcher :(
I have a chieftec  630W power supply and before I did not have any problems with overclocked GTX480, which consume more power than 970 ... I should RMA? When purchased this card I assume I will be able to turn clock higher. Any ideas?

    What model power supply do you have. Just the brand doesn't tell much, and some of Chieftec's units aren't even 80 Plus certified which is bad. I've honestly never even heard of that brand before until searching their name. I would get a unit from a reputable brand that is at least 80 Plus Bronze and try that first if that's all you can afford but I can really only recommend at least a Gold rated unit for gaming rigs. Crap power supplies will easily be the cause of issues like these.

    Damn, its Thermaltake Smart SE 630W, I forgot that I bought a new :-) I did not have any problems with the gtx480 heavy overclocked who has a bigger TDP. This is strange

    Heard a lot of bad things regarding Thermaltake...Since these errors started, did you try putting back the 480 to see if it still works without issue?

    Yes, i was playing all the time Battlefront and that was a reason to change my gpu. After purchase the problems started. For long time I forgot about this issue up to the point where I bought a 144hz monitor :( I can borrow in next 2 weeks corsair 850w for tests.

    Alright, yeah I would try another PSU first if you can. If the issue still happens then I would RMA the 970.

    To check the power supply you can go into the motherboard BIOS/UEFI and look for "Hardware Monitor" or "PC Health Status" and look for a live readout of the 12v rail of the power supply. The 12v rail should read between 11.4v and 12.6v (+/- 5% of the rail's rating). It should be stable, with little to no fluctuations. A fluctuation higher than .1v, or more often than every 10 seconds could be an indication of an issue with the 12v rail, and you should consider having your power supply tested or replaced.

    My 12v rail in uefi shows 11.985v - 12.065v. And my gpu vcore  is 1.225, looks normal

    You said you can borrow a Corsair. Please do that and see if all is well.

    Make sure that you remove All the Thermaltake Smart SE 630W Power Cables and use Only the Corsair Cables when you test.
    Do not use ANY of the Thermaltake Smart SE 630W Cables on the Corsair Power Supply.

    Is this while idling?

    There is a strong possibility that your PSU is sagging at different variables to cause it to black screen (just like evgatech_ray mentioned), during intense scenarios while running the application. During idle it might look normal.
