Don't know what to tell you. I've got a couple of games on here that cause the problem consistently. Other (most) games never do it at all. For the games that do it consistently, they stopped doing it when I disabled G-SYNC. As this was done at the suggestion of someone else with the same issue over in the GeForce Forums (
https://forums.geforce.com/default/topic/968520/pc-games/gsync-feature-causes-black-screen-amp-pc-freeze-when-exit-some-games-/), I'm now pretty sure this isn't an isolated case (bad monitor, cable, etc.) The only thing I've noticed for the games that repeatedly cause it is that they don't have a setting in-game that allows for setting the refresh rate. Most games allow me to crank it up to 165 Hz.
Do you have the game Braid on Steam? If so, try it out and see if it works fine for you. It is an older indie game, so I figure it might fly under the radar for some/most these days. On the other hand, Dark Souls III is a more popular and recent title, and it does it as well.
