Don't know what to tell you. I've got a couple of games on here that cause the problem consistently. Other (most) games never do it at all. For the games that do it consistently, they stopped doing it when I disabled G-SYNC. As this was done at the suggestion of someone else with the same issue over in the GeForce Forums (https://forums.geforce.com/default/topic/968520/pc-games/gsync-feature-causes-black-screen-amp-pc-freeze-when-exit-some-games-/), I'm now pretty sure this isn't an isolated case (bad monitor, cable, etc.) The only thing I've noticed for the games that repeatedly cause it is that they don't have a setting in-game that allows for setting the refresh rate. Most games allow me to crank it up to 165 Hz.
 
Do you have the game Braid on Steam? If so, try it out and see if it works fine for you. It is an older indie game, so I figure it might fly under the radar for some/most these days. On the other hand, Dark Souls III is a more popular and recent title, and it does it as well.

I don´t have gsync in here. Just a pretty ordinary ASUS 60mhz FullHd monitor (mx279) running at 1920x1080 and the black screens happens randomly with some games (Only when i exit the game). I have just made a clean nvidia driver install 375.76 and will see what happens. Will let you know if the issue is gone or is back ... fingers crossed

Interesting! Maybe we're dealing with two distinct but similar issues? Does my putting the computer to sleep trick work for you? Also, you say the issue is random for you. For me, it can happen every single time with a couple of games.

Tried the braid demo. No problems here.

No idea. Maybe something different in demo version? Something else different between our systems that we're not accounting for? Doesn't change the fact that, in my situation, disabling G-SYNC for the affected games stops the issue for me.
 
BTW, can add Final Fantasy XIII-2 to the list of games that do this. Yet another game where I can't specify the refresh rate in the game, so that theory still holds.

Will test Braid tonight, as i have it on Steam. One thing to note is that i´m using 1ft class games like BF1, Mad Max, Doom and in this games, i don´t see the black screen. I see much more black screens in indie games.

I think i might have had black screen triggered in dark souls 3 when messing with settings it will flash, if you set refresh rate by aplication instead it will stop giving blackscreen, if its set to force max refresh rate it will cause blackscreen.
 
Not sure if this same issue.

I will try leaving G-SYNC on and changing the refresh rate setting in the NVIDIA Control Panel for Dark Souls III and see if it still does it. I believe the default setting for me is indeed to force max refresh rate. This makes sense when combined with my theory about the affected games not allowing for setting the refresh rate via the in-game settings. Thanks!

i just go for aplication that gives issue and set preference by aplication instead for the game that gives issues btw.

It can be a dp cable issue, try to find it on google with 20 pins issue...
I have the same issue with 2 cards, I just bought a DP cable to try to fix it (I have this problem from 1 year).
