Lately I have noticed a difference in both gameplay and video. I noticed a lot more stuttering in games and videos and my videos have start tearing both horizontally and vertically. Noticed some tearing in Witcher III as well with vsync turned on. Last night I was streaming a V8 Supercars race in browser. Usually no issues but the stuttering and frames skipping was horrid. Make it difficult to watch. I was playing minecraft as well. All the sudden everything freezes and a system waiting icon appears on the mouse pointer but yet I can still hear audio from the race and hear myself walking around in minecraft as I use the keyboard. Then both monitors went black and both reported no signal on both HDMI or DVI. Only way to fix this was a hard reboot. 
Framerate drops I have noticed. 
Minecraft (unlimited frame option) Went from ~500fps -> 55fps (not that 500fps is needed or applicable, its just to show a comparison) I can no longer keep minecraft stable at 60fps
World of Tanks 60FPS(vsync) -> ~50fps(vsync)
Witcher III (High Settings) ~50fps -> 25fps
Increased Horizontal and Vertical Tearing on both Blu-ray, dvd, and video files from both my ssd (Raid 0 array) and my Hard Drive 
I have also noticed some videos are also out of sync where  the audio is ahead of the video itself.

CPU voltage reports 1.2059v (Core Temp 1.4)
GPU under load reports usually around 1139mv (Evga Precision X 16) 

Firestrike benchmarks seemed to run fine. Final score was in the lower end of town compared to other valid results.

I am not sure where to go from here. Any suggestions? Thank you!

Check and make sure your cpu isn't overheating while you're gaming. Make sure v-sync is to application controlled inside the nvidia control panel. Try retesting your games with your gpu in debug mode (#3). Run your cpu at stock clocks.

I will attempt debug mode and attempt a clean install of fresh nVidia drivers tomorrow.
CPU I have never seen go above 52C with anything I throw at it so CPU is good. 
v-sync is set to Application Controlled.

Alright so Debug mode is causing a bit more trouble. Where the GPU had no issues rendering a frame it now struggles to keep pace with FPS fluxuating quite a bit. Noticed with a lot in minecraft where a larger spawn structure was no issue 2 weeks ago where now the GPU struggles to stay constant jumping from 20FPS - 60 FPS. With higher temps noticed as well compared to normal mode. 10C higher about. One night the GPU seemed to have issues displaying pictures from facebook where different blocks of the image would should up scrambled like a block puzzle and took a few minues for it to display correctly. Thing is that this does not happen consistently, and seems to worsen the longer I run the computer. Sometimes I have issues right from booting the computer sometimes no issues at all.

Install driver 347.88. Follow #2 listed here when installing driver 347.88. Let me know how minecraft & wot runs on driver 347.88.
