I have formatted and reinstalled Windows and stuck with the 372.90 drivers. So far, I've not noticed issues (in basic benchmarks). I plan to fully test the system.
 
Is there a good way to replicate playing something like DOOM or BF1 without having to play it (not Heaven or Valley)?

I've played a few hours of CS:GO and noticed no change, but then CS:GO mostly worked beforehand. I've also tested 30 minutes of Fallout 4 (Ultra, UHD) and it seemed about as I remember.
 
I don't have time to test games at the moment. However, I did go out and leave RealBench running on a stress test. I chose 8GB RAM for 4 hours - it passed without issue (this was a problem beforehand).
 
This is very, very odd.

Some graphics drivers can cause a black screen at the end of a realbench run. Looks like 372.90 isn't one of them.

All of those have passed. Oddly, since testing a new driver, Heaven gives a 10fps average increase compared to the current driver. I'm not sure whether this is a hardware of software issue and that annoys me. The black screen in two games does seem to indicate hardware - and Jacob's explanation in the thread shows why this might cause issues only under certain circumstances.
 
I'll continue to perform tests as I've lost faith in EVGA's RMA process after seeing the number of people who received defective cards after sending theirs back.

I know you black screened when running in debug mode. Try using msi afterburner to underclock your card as far as you can to see if it helps get rid of the black screen. If underclocking doesn't help try running your gpu fans at max speed to see if it helps get rid of the black screen.
