I have a GTX 660, and recently I started having a weird problem where my monitors will go black, and then sometimes go back to normal, or they will go black and then say HDMI/ DVI (Dual monitor) and my computer stays on, but I can't hear sound or do/see anything. Also sometimes while I am watching a twitchtv stream, it will happen, and when it comes back the stream is all green, and Blizzard's battle.net program will no longer open. I mostly play WoW/overwatch/watch twitchtv. This problem also happened on path of exile in the past.
 
It's a new card (the old 660 had the same issue though). So here's what typically happens.
 
I open overwatch/wow on my first monitor, and have a twitch stream on my second monitor. Anywhere from 5 minutes to a few hours go by, and I get these different scenarios that mess my PC up. I troubleshooted this before, and my temperatures were fine. I found some posts online with similar problems, where they recommended increasing the voltage by 10% in Precision, and lowering the clock speed. I tried lowering the clock speed, but I will try increasing the voltage as well I guess. I constantly keep track of temps with speedfan, and the GPU will crash sometimes at 60C, and the CPU will be at 40C.

I also get a notification that my drivers crash sometimes when I get this problem, and it tells me it was successfully restarted, or whatever it says.

'Speedfan' has been known to be buggy and cause problems. I would remove it first and see if you have any improvement. If you need to monitor your temperatures then I would recommend MSI Afterburner. It's what I have been using for years without any problems at all. Now you can stack the graph from each Core into a single panel and assign each Core a different color to monitor each Core. If you have PrecisionX installed you will need to remove it entirely before using Afterburner.

I would suggest using GPU Shark to watch your Temperatures.
To set the Fans only on your GTX 660 just use EVGA PrecisionX 16 Version 5.3.11
Set the Fans how you want them and then EXIT EVGA PrecisionX 16 Do not set it up to run with Windows Start up.
It only takes a second to open EVGA PrecisionX 16 and set the fan or you save a profile then select the profile.
Also you can use NVIDIA Inspector to set the fans. For as old as the GTX 660 GPUs are I would not overclock them.
To set the Fan in NVIDIA Inspector Click the Button at the Bottom Right Show overclocking and then set the fan.
You can also view the GPU Temperatures with NVIDIA Inspector
As Zuhl3156 above said stay away from SpeedFan.

I had problems with this before I used speed fan. I went through EVGA customer support VIA e-mail and I did a clean install of drivers with a program he had me use as well. The temperatures aren't the issue but I will get rid of speed fan.

Which drivers? I use 372.54 because the newer release causes many people issues. Are you using GeForce Experience? If so, I would remove it. Also known to cause issues.

I just always download the latest drivers(372.70), and geforce experience is installed ATM.
My power supply is cooler master, I can't find my box/order history but I know its at least 600+watts, around 2-3 years old. ( and I opened my case to check for a serial number and there isn't one, at least not in the side facing out)

Lowering the clock speed/increasing the voltage hasn't helped? What motherboard are you using? Is the motherboard bios completely up to date?

Dump GeForce Experience and go back to Driver 368.81
