Ah apologies, I misunderstood, I missed the part where you mentioned you plugged the monitor into your on-board graphics to get your system working.

I'm not trying to pour salt on wounds, EVGA is a great company and everyone hits a snag here and there. I really enjoyed my FTW until this issue arise among the community, including myself. After reading many reports of RMA's disasters, I didn't feel confident in my issue being resolved, or knowing exactly what the root cause is. Instead we were left to blindly hope that our products would be ok. Ultimately I returned mine and purchase a Asus Strix 1080 OC, out of the box it boosts up to 2068, and stabilizes itself around 2050 without touching anything. Games play flawlessly and I'm not clinging my mouse hoping it's not going to crash this time. Why am I talking about another card here? I'm bringing a point that this is how the FTW should have been, enjoying the games, boosting to your peers I have a EVGA.  I'll try EVGA again in the future, but I hope they listen to the community and offer more transparency and honest communication in the future.

Got my RMA replacement yesterday. Tonight it crashed for the first time after about an hour on Witcher 3 :(
 
First thing I checked was the chips on the back to make sure they were the newer '66' variety as I thought that was the issue but I guess not. 
 
I'll be back on the phone to EVGA tomorrow. Really not happy

Received my RMA FTW Hybrid card today. I haven't even installed it into my PC. As I was unwrapping all the plastic and port covers I noticed that there is no BIOS switch to toggle between master and slave BIOS! The marking is on the card but there's no switch! I emailed support to request another RMA. This whole process; black screen and fan issue plus this now is so annoying.  Stuff happens beyond EVGA control I get it. Maybe it is a sign that I should stick with my 980 Ti.

Just received my RMA today, here is what I know so far:
 
IMPORTANT! EVGA Precision XOC does not fully remove itself after uninstalling/reboot - I had to delete registry settings in order to clear out my existing profile and allow entry of new Serial #.
 
* Card does not appear visually to be any different from the existing - maybe a little darker color on the heat-sink.
* After DDU, re-install drivers, and XOC boosts to 2025Mhz/5005Mhz on default profile.
* Running 8 hours of Heaven Bench before anything else, and so far no problems after an hour with temps 50-60c (MUCH cooler than before).
* ZERO COIL WHINE. 
* EVGA Customer Support FTW!! (groan).
 
Fingers crossed.
 
NOTE: I have never had any coil whine with the past 3 cards (MSI GTX 760, MSI GTX 970, EVGA GTX 1080 FTW) on this MOBO/PS (Old Gigabyte p67+Thermaltake 850W modular). However my old MSI GTX 970 sings like a bird in the machine I built for my Nephew which is a newer ASUS Z97ar/i5-6700K/1000W - go figure.
 
EDIT: 'Chips' on the back are 65 lot, my existing card actually had 66 lot.
 
UPDATE 1: It is clear this card is clocking down at much lower temps than my previous one - 1960Mhz@60c whereas my existing card would hit 2020Mhz@75c. That is not a complaint just an observation - it is still astonishingly fast.

I placed my RMA for this issue almost a week ago and it doesn't seem like the replacement has even shipped yet.  Is this typical?  It's a Hybrid, not sure if those are still having stock issues and if that's affecting things.

Contact support directly, they will sort everything out.
 
EDIT: It took about a week for my RMA to ship also, so that sounds about right.

Pretty much this. A simple "This is the issue and it's been rectified from version XY" would put a lot of minds at ease. However, EVGA refusing to publicly state the issue makes me think this is a problem with all FTW models produced up to a certain date. I've spoken with EVGA and found them very helpful, and one supervisor did put my mind at ease, but it's not what I expected after reading countless external forum threads about EVGA's excellent hardware and support.
 
At least MSI admitted there was an issue with certain 1070 cards and issued a partial recall.

I feel quite anxious about the new RMA card, the serial number is 1612536286802389 which is prior fromer card 1612536286802508 having the black screen + 100% fan issue. It was said that a hardware fix was put in place after the 30th of August (in your official forum) and I get a fully sealed card from prior that date. 


How can I make sure that the new card is bullet proof ?
