I had the exact issue when I first installed my 1080 FTW. I would be in the middle of playing Team Fortress 2 or Overwatch and all of the sudden the system would lock up, both screens would go black and the sound would stutter on the last sound played. The fans would rev up to the point where it sounded like a jet was taking off. Only way I could get out of it was holding down the power button to shutdown. Tech support suggested I use DDU to completely remove all traces of any video drivers and reinstall fresh drivers.
 
After that the problem went away. Unfortunately, after updating to the latest drivers the other day the problem has come back. Happening twice last night. Once after a few hours of play the second after only 30ish minutes.
 
My specs:
MOBO: Gigabyte Z170X Gaming GT
CPU: Core i7 6700k 4ghz (Corsair H100i liquid cooler)
GPU: GTX 1080 FTW
Memory: Corsair LPX 32gb 3200mhz (2 x 16gb)
HD: Samsung EVO 950 (M2 512gb)
PSU: EVGA P2 1000 (80+ platinum)
 


It is definitely an unstable overclock. Just test the games again using debug mode in the Nvidia drivers. Just go under help and select it.  Debug mode will remove the factory overclock temporarily.  For me I found out that fixed my issue. It's not right that I paid extra to have a factory overclocked card for it not to work especially since my temperatures are within operating limits. So I am doing an RMA. Not sure if I will go back to EVGA or not.

Just a FYI to everyone monitoring this thread... the OP's issue was resolved by rma'ing the card.

It certainly was. Had a great weekend playing hours of Just Cause 3. Such a great game with everything maxed.

FWIW - The new drivers came out with just enough time for me to test with before I returned the old card. I used the anniversary version and experienced the same crashes.

What I will say from following this thread is this. If you are experiencing any issues then start with the basics before going down rabbit holes of crazy troubleshooting ideas. Read and follow Sajin's stickied thread.

I just received my 2 1080 FTW's today and installed them.  The 372.54 drivers black screen, hard lock my system and kick card 1's fans to 100 percent after about 5 minutes in a game.  But 368.81 drivers and msi afterburner run just fine.  No issues at all.  So I guess for me anyway it was a driver issue.

I just got my gtx 1080 ftw and I have the same issue as you guys. I am really sad about it... I think I will just ask NCIX to replace my card, given that I just got it...
 
Note: I heard it might be a PSU related problem. My psu is kinda old (750w NAXN), it might be the problem ? I am not really advanced with the hardware installation, it is my first time playing with the parts of my PC... AT first I tough I plugged the wrong PSU cable in my GPU, but I think that's not the problem... I plugged 2x (6pins + 2pins) and the card look like it run just fine, until I get that black screen...
 
Anyway, I might just ask for another card... :(

So sorry to hear about that. The problem lies with an unstable overclock. Basically EVGA did not test their cards before going out of the factory. They might have done a sample test where they took maybe like five cards out of every hundred and tested them but that's about it. The card comes with a pretty aggressive overclock and it's failing. It has nothing to do with your PSU. Go ahead and do a test. Go to your Nvidia settings and change to debug mode. This will put the card into reference clock settings. You will find that everything works like a charm.

Have the Same issue with my New 1080 FTW. Went through all the troubleshooting, 4 revisions of drivers, tested on both win 7 and win 10. I noticed when not in debug mode the card would OC itself to 2025mhz which is well over what it is supposed to OC to. In debug mode it would still sometimes hit 1900+mhz. As most people have done I immediately set an aggressive fan configuration starting at about 70% which keeps the card below 60C.
 
Debug mode caused a different issue for me. Instead of the driver crashing and all screens going black, only the primary screen the game was on would go black. 
 
RMA is already in process. Don't waste your time troubleshooting beyond the basics. As this thread grows it is obvious that it is a hardware issue. Would really be nice if EVGA would at least acknowledge the issue and not continue having people waste their time with testing that will accomplish nothing.

I am not a pro with oc and stuff, so I can't really tell what's the problem here... I already started the RMA with NCIX. Could you explain to me how to put the card in debug mode ? I will make some test. Thx for the reply :)

just to let you know guys, I sent back my card to RMA. I ordered a new PSU too (Supernova 750W). I hope the problem is not my motherboard (GIGABYTE GA-Z170XP-SLI), but I don't think so, everything were working fine till I changed my GPU...
