If you purchased your card from the EVGA store, then register your card, go to the site here, and redeem your code. If you purchased it from another retailer, then you'll need to contact their customer support for your code.

Don't forget to claim the usable codes at http://www.geforce.com/GetMGS  (Generic for everyone)
https://redeem.geforce.com/en-us/?campaignID=MGSVPHANTOMPAIN (specifically to redeem codes in the US)
 
Once that is done, you should get another code that can then be used on Steam to claim the game for your account.
