keyboard / macro and button_down/button_up

Trying to capture [alt+tab] I ran into a problem.
the "keyboard" map function can't capture this, but does have the behavior button_press = keys(s)_down / button_release = key(s)_up
The macro function can capture it (with a little creative editing), but has the behavior button_press = play keystroke sequence
 
is there any way to beat the OS preempting the "keyboard" capture?
if not, is there any way to bind the macro so specific parts play at button_down / button_up?
 
the latter would be powerful in gaming scenarios as well... prep sequence runs on down, hold till opportune moment for finish sequence.
side note... there seems to be no "repeat macro while held" is this on the roadmap for future updates?
