gtx 660 core clock won't go over 324 mhz

I'm relatively new to getting into the specifics of how the cards operate, so forgive the ignorance.
I have a GTX 660, and the core clock won't go past 324 MHz, even under load(I monitored this both while running 3dmark and while playing a game, using GPU-z). I uninstalled all of the drivers, reinstalled them with a clean install. I downloaded nvidia Inspector, and I noticed that the voltage is at .875(I read that this should probably be around 1.14v?) and the power % is hovering around 330%(im suspecting that this isn't good). The temperature didn't get above 60 after about an hour of playing BF3 on ultra settings(in multiplayer--I didn't notice much stuttering or frameskipping while playing). I tried to adjust the clock settings to what's supposed to be stock speeds with nvidia inspector, but I was only able to get the memory clock back to stock speed, and only up to the P2 state(no adjustments held in the P0 state). When I tried using the Kboost feature in Precision X, the voltage didn't change at all.

My system is pretty old, and I'm in the process of building a new one, but I got this card to put into the current system as I accumulate parts for the new rig.

What could be wrong?

    Welcome to the forums.
     
    Forget about NVIDIA Inspector, lose it and just use GPU-Z and either EVGA Precision X or MSI Afterburner (which supports on-screen display for 64-bit games/apps, while EVGA Precision X does not).
     
    Uninstall the NVIDIA driver again in Safe Mode using Display Driver Uninstaller (link), choose Clean and restart (Highly Recommended)..Then clean install the driver using the Custom (Advanced) option, choosing only to install PhysX and HD Audio..Restart again.
     
    Open GPU-Z and click the "?" beside "Bus Interface" and start the Render Test which should make the card run at it's Boost Clock which can be seen by opening GPU-Z's "Sensors" tab..When you stop the Render Test your card should shortly downclock into power saving mode..How many MHz is it boosting to and how many MHz at idling now?
     
    If you open the NVIDIA Control Panel -> Manage 3D settings -> Global settings and set "Power management mode" to "Prefer maximum performance" and reboot, your card should be prevented from going below it's Base Clock speed, but then boost during the Render Test (or any stress test/benchmark), and then shortly return to Base Clock when you stop the Render Test.
     
    You can see this all happening in EVGA Precision X or MSI Afterburner too, and in On-Screen Display if you set it up..You can use EVGA OC Scanner (link) as a stress test too.

    Just to be on the extra safe side you should uninstall precision/afterburner before you use Display Driver Uninstaller.

    Looks like your card's VRM's are toast. Have you tried reseating the card and power connections?

    Yes, I have. I got the memory clock to adjust to the stock values and remain there (until I reboot), but the core clock values wont change no matter what. Is this card dead?

    The process of getting through tech support to get the RMA approved was great..no hassle, efficient, and fast. I'll update once I receive the replacement.

    I got the RMA replacement card in, and everything works fine! EVGA even sent back a slightly better version of the 660 ! I was pleasantly surprised and excited.  They definitely just got some points in brand loyalty with me...
