I purchased an EVGA 240gt product on January 4th 2011, sent the rebate, got denied of my $40 rebate.
Here's a part of an email I recieved from the EVGA rebates after mailing my rebate form:
"
Your Rebate was not approved for the following reason(s)

- Signed Rebate Form is missing
- Purchase date 01/10/2011 is outside offer's time frame

Our decision to not approve your rebate claim was based upon examination of the documents that we received from you.
If you wish to view all documentation you may simply click on this link

However, the rebates sponsoring manufacture/reseller has authorized us to offer you one additional opportunity to re-apply for this rebate claim.
There is only one exception to re-applying, and that is, if your rebate claim was denied due to being outside of the allowable time frame, then
unfortunately you are not eligible to re-apply.
"
Now I
had a photocopy of the original form and other documents that were unsigned, and it's quite possible I forgot to sign the form or I simply made a copy prior to signing the documents. But the fact remains that I purchased and paid for my product on JANUARY 4, 2011, that's what it says on my paypal transaction. The purchase date was somehow fudged up to January 10, 2011 by either canadacomputers.com (where I bought the card) or EVGA.  Now by all means the quality of the product is excellent and has far exceeded my expectations. However, those who wishes to save a few bucks with the rebates should RUN RUN RUN. Quite possibly they may change your purchase date simply to disqualify you:

"There is only one exception to re-applying, and that is, if your rebate claim was denied due to being outside of the allowable time frame, then
unfortunately you are not eligible to re-apply.
"

here is paypal with  personal informations removed:

Wonder why they dont list any Expired GTX 240 rebates
http://www.canadacomputer...item=&x=32&y=5

And under all Rebates it doesnt show any GTX 240's with a 40.00 rebate ?(just 20.00 and 30.00)
http://www.canadacomputer...item=&x=41&y=6

Maybe im searching the site wrong.Do you have a link ?
Also what date was your paypal actually deducted the money(not sent date 1/4/2011 but the date the Store took the funds )? If the item was not processed by the company until it had the item to ship they may have not charged(taken the payment) until the 10th.Just a thought

Strange GT or GTX ,not that it mattered as i searched all EVGA rebates current and expired, that there site doesn't show that form that your clearly have a link to.What was the Actual date that Canadacomputers took the funds from paypal? not the date you submitted payment as you have shown above? Was it on back-order at the time?
Either way im sure EVGA will help sort this out .
