LockedUrgent Help, GTX 750 Ti SC Crashing on Games.

Hello, I am new here.
I have recently bought a new EVGA GeForce 750 Ti SuperClocked 2GB
I installed the Card,  Latest drivers from Website. everything was working proper but whenever I start the game it works fine till Main Menu but as soon as I press Play button my monitor goes Black and display a message "Input not supported" and then PC restarts.
It is a brand new Card, I did all the possible things to fix the issue but nothing worked.
 
Things I tried:
1. Reinstalled Latest Drivers
2. Installed old Drivers
3. Removed old Drivers using Driver Sweeper from Safe Mode
4. Reinstalled Windows
5. Tried changing settings from BIOS
6. Update BIOS
7. Disable all Graphics settings to Low in Games
8. Roll backed drivers from Device Manager
9. Changed Monitor Refresh Rate
10. Disable 3D Settings from NVidia Control Panel
11. Increased Fan speed using PrecisionX 16
12. Tried different Monitor
 
None of these worked, Later I got the BSOD so I used BlueScreenView to check dump and found out the "dxgkrnl.sys" crashing.
I have posted the Dump below.
also I want you guys to know that everything else is working perfect except the games.
infact the games are working too till the Main Menu but as soon as I press Play it crashes and PC Restarts with Blank Screen and on Windows Startup I get message of "windows has recovered from an unexpected shutdown"
Please help me, I have bought this new Card and cant even play a single game.

Intel Core i3-4130
GIGABYTE B85M-D3H
HyperX Blue 4GB DDR3 1600MHz DDR3
Western Digital 1TB HDD
Intex 450W PSU
EVGA GeForce 750 Ti SC 2GB GDDR5
 
Games I tried to run:
 
Grand Theft Auto IV
The Sims 4
Far Cry 3
Need for Speed Most Wanted
Counter Strike 1.6

    checks the memory first, remove and put to back, or change it to another memory, if it does not work, try the power suply, for me it is small, uses one of 600 wast
    PS: memory or power suply (for my power).

    es pocible que sean las memorias podrias cambiarlas o poner otras ,lo otro puede ser el power suply le esta faltando corriente ,

    I only have one Memory installed, still I reinserted the memory but it didn't worked also I don't have extra power supply so for that I have to buy a new PSU. Any other possible suggestion which could fix my problem.

    Test the card in another computer to see if you can replicate the issue. If you can't replicate the issue in another machine replace the PSU in your machine with a higher quality unit.

    First: It is good to you to put another 4GB memory, as model to have a dual channel.

    Second, check if the 6 pin pci e the works, change it for another 6 pin pci e, the psu.
    for my needs more juice the Power Supply.

    Update: So I have just now updated to new version of GeForce Experience and now whenever I start any game, my monitor goes RED and System crashes.

    Can any one help me here please. I have been trying to fix this since last month.

    Did you try testing the card in another computer like I recommended? If not, try downclocking the card -105 on both the core and memory using MSI Afterburner then retest the system. Report back with what happens after downclocking the card.

    Remove the drivers while in Safe Mode and before restarting, delete the following folders:
     
    -C:\NVIDIA
    -C:\Program Files and Program Files (x86)\Nvidia Corporation
     
    Once those 3 folders are gone, restart and try installing the drivers again using the Custom(Advanced) option.  Only install the driver, PhysX, HD Audio (if you are running audio through HDMI or DP) and before clicking next make sure to check the box next to Perform Clean Installation.  Restart.
     
    This should discount driver software being an issue.  Let us know if you can do what Sajin suggested as well.

    Running into the same issue 
     
    My computer has the following set-up:
    Graphics Card:EVGA Geforce GTX 750Ti
    MOBO:Gigabyte F2A85XM-HD3 Rev 1.0
    CPU: AMD A8 - 5600K APU
    MEMORY: 8GB RAM
    Windows 7 professional SP1
     
    Followed your recommendation on delete three folders in Safe Mode. Restart and install drivers as per instructions and restart. 
     
    None of the games will work in 1920 x 1080 mode. The initial re-install brings everything to 1280 mode not 1920 mode. At 1280 the game loads. The moment you set the video to 1920 and try to play the screen goes black or sometimes orange and Windows restarts. 
     
    PSU on this computer is currently good to max of 470W
     
    Somewhat discouraged because this is a new install for the GTX 750Ti and the MOBO was installed in February. 
     
    This GTX 750Ti was recommended by both EVGA tech support and the vendor as a video card compatible with the Gigabyte mobo and the AMD A8 5600K. Yet after two days of trying nothing seems to resolve the problem. 
     
    Any recommendations, solutions would be greatly appreciated.

    Update: I have recently bought new Monitor BenQ RL2455HM and also did a Clean Install of Windows 8.1 but having the same problem.  I Installed Far Cry 3 and did all the Video settings to High but as soon as I click Play button, Screen goes Blue and PC Restarts, Please help me.
    I tried DownClocking to -105 but that didn't helped either.
     
    Things I haven't tried yet:
     
    1. Installing Drivers in Custom options, which is recommended by
    2. On Different PC with good PSU, I have a friend where I can test but he have the same Config (i3 with 450W PSU).
     
    Please tell me what to do.
