Titan Z problem

Hello all,
 
I recently purchased an EVGA Titan Z. I installed it into my machine, installed the drivers, and started playing my games (Witcher, etc). Here is the problem: about two hours into playing, the cooling fan went berserk, immediately running at 100%, the screen goes black and is unresponsive.  Occasionally I do hear a windows error sound and the machine does still appear to be running as the SSD light is still flashing like everything is fine.  I immediately turn off and turn back on (as reset does not help slow down the fan or get the black screen fixed) the machine and check the temp of the card.  It says 42 so it does not appear to be a thermal issue.  It will even do this every day or two when everything it is at idle.  Sometimes it does it when I am surfing the web.  I really don't know where to go from here.  I updated my bios.  I updated the card drivers.  No avail on anything I tried.  Here are my specs:
MB: ASUS P8Z77-V LK (latest bios)

CPU: i5-3570K
RAM: 16GB
SSD: 240MB
PSU: Sparkle 850 gold (bought 1 year ago)
OS: Windows 7 pro 64bit.
Driver: 358.91
Overclocking: none
CPU is on watercooling.

I am at a complete loss.  Any help or ideas would be great.  Thanks!

    Try using driver 347.88 to see if it helps fix the problem. If you get the same problem using driver 347.88 try testing the card in another machine.

    Loaded in 347.88 after cleaning out prior driver with DDU.  Same problem after about 1 hour on web-Youtube videos.  Fan goes full speed and black screen.  I don't really have another computer just laying around to try this card in.  Oh, one more fact that may help.  This is my second Titan Z as the one I had prior did the same thing.  This is not something that just happens right out of the gate.  It happened on my first Titan Z after about 2 months and then got more and more increasing.  The same thing is starting to happen with this one.  Any ideas?

    It sounds the GPU driver is crashing.
     
    When you crash in Witcher 3, does the game continue on background (can you still hear the sounds like it's still going)? If yes, try removing all monitoring software like Precision and Afterburner from your PC.

    Maybe your psu is killing the cards?

    Ok, got it figured out.  The Titan Z is very picky with it's power supply.  I went and got a new one (Corsair RM850i) and it runs with no crashing.  I thought Sajin might be right when it crashed (ran the fan berserk) and when I reset the computer it started in berserk mode.  I turned it off and started it again with the same problem right out of the gate.  I turned it off and unplugged it for 12 hours and the same problem as soon as I turned it on.  I simply cycled turning it off and on 10 times and it started up okay on the tenth.  That seemed to say the Titan did not like the power it was receiving.  One new power supply later and no problems.  Thanks for the help guys!!!  I'm glad you all are around!!!
