RMA process for my EVGA GeForce GTX 770 Classified ACX 4GB purchased in 2014?

Just after some information on the RMA process for an Australia resident beyond that found at 
 
Bit of information on the issue with my card below.
 
The Problem:
So for over a year now my EVGA GeForce GTX 770 Classified ACX 4GB card has been one of the bane's of my existence. Issues include:
  • Screen constantly going black (display driver crashing). Occurs at least once a minute for quite a while after the computer is switched on. Often requires a restart as the driver does not recover, which is more so required during high load application such as gaming.
  • Get squares of patched colours that appear all over the screen as part fail to render.
  • Constant crashes during gaming
  • Computer stalls on startup. i.e. starts, then turns off, then back on and starts up normally (occurs over a matter of seconds)
The above doesn't nearly begin to adequately reflect my frustration. I finally have time to deal with a return process. I am not looking to discuss the issues really as I have troubleshot them to death already.
 
Purchased the card from PC Case Gear in Australia.
