Can someone direct me to some step by step instructions on how to setup a single GTX 660 for 4 monitors. In the specs on the card it states that it supports up to 4 concurrent displays.

I had no problems with running dual monitors. I purchased and added DVI-D splitter cables, one attached to the DVI-D port and one attached to the DVI-I port on the GTX 660 card.  I added the two additional monitors today and they all power up and have output. However, they are cloned.
Monitor 1&3 = main Windows display
monitor 2&4 = extended display.

The splitter cables are Cables Unlimited PCM-2260 DVI-D Splitter Cable (in case that's important).

When I run the nVidia control panel it only shows 2 displays on the list (both are checked). I've gone into the Windows control panel but it only shows 2 displays as well.  I'm not a gamer (except for an occasional foray into Guild Wars and I have a laptop for that). I'm really hoping that the "4 concurrent displays" wasn't having to use the HDMI port and whatever other port is available on that card.  None of the monitors I have are setup for HDMI or anything but DVI-D and VGA.

Thanks,
Bill

The 4 monitor capability is not via splitting cables.  The 4 monitor capability via 1 monitor on DVI-D, 1 monitor on DVI-I, 1 monitor on Display Port, 1 monitor on HDMI.  I have set up this exact same setup on my brother's desk with 1 GTX660 because he is a day trader.  We did this card so that we can add more cards for more monitors once he actually starts making some money.

Thanks.  I just bought adapters (DisplayPort to DVI and HDMI to DVI) which will hopefully work since none of my monitors have DisplayPort or HDMI ports.   I'm not a Day Trader but have been running systems with either 2, 3 or 4 monitors for the past 10 years.  This card has been the most expensive and confusing one I've used so far.  Ahh well, live and learn.  Like your brother (and I wish him luck), once I have money coming in again I'll probably go for a second card. After the amount of time I've invested, and everything I've learned (with the forums help) about this card I don't want to change again for as long as possible.

Thank you.  If I read it correctly, I need 2 adapters (one HDMI to DVI and one DisplayPort to DVI) in order to use the other 2 monitors.

Correct.  What I have done with my brother's system is actually get cables that have the adapters builtin.  I shop at Monoprice.com as they have awesome cables for good pricing.
