Is my gpu dying?

I have had this gpu now, EVGA gtx 570 for 6 years now, and I think it's finally giving up on me, I simply can't use the drivers anymore because in windows a blue screen shows up everytime I try to turn my computer on, safe mode and booting without the driver installed works fine. In linux I installed the driver too from Nvidia and it doesn't work also when I try to start it up it freezes up and some blocks of colors pop on the screen.
 
Now I'm worried because I'm looking to buy a new graphics card EVGA Gtx 750 Ti
but what if the same problems occur with this new GPU? what if it isn't my gpu thats dying but the motherboard or the PSU.
 
So my question is how can I isolate the problem, I already ran 9 passes on mem test and found no errors. I already tried changing my gpu to another pci slot and same problem happens. Also when the problem started, with the display driver stopped responding and has recovered error and sometimes a blue screen afterwards. I toned down the resolution in fallout 4 and was able to play for a few days with no issue but it came back and now with a vengeance I cant boot anymore into windows the problems I described above

ASUS p8p67 motherboard
i7 2600k cpu
earth watts antec 750w psu

    Hello,
     
    by the information you've provided I would say its likely a gpu issue.
     
    To test your PSU, go into your motherboard bios and monitor the 12v rail reading.
     
    If its lower than 11.5 or higher than 12.5 or if its fluctuating all over the place it may be time to purchase a new PSU.
     
    12v rail issues can cause problems like the ones you're experiencing however more than likely its a GPU issue.

    Well I checked it out and the 12 rail is reading as 11.9 and 12 is that ok?

    might be a temp issue...
    how long since you did the thermal paste on the heat sink?
