How to check if I have 970 FTW or 970 FTW+?

How can I tell which card I have?
 
I bought it in February 2015.

    On the back of the card near the warranty sticker there is another sticker with either 04G-P4-2978-KR(FTW) OR 04G-P4-3978-KR(FTW+).  The last two letters may be different depending on the place of purchase.
    04G-P4-2978-KR - FTW
    04G-P4-3978-KR - FTW+

    You can also do a gpuz verification, and it will give you the part number.

    the FTW+ comes with a backplate, the FTW not.

    Also on the side of the GPU where you plug in the power cables, it will say either FTW or FTW+

    Another way to tell them apart is by their display outputs. The FTW+ will have 1 DVI, 1 HDMI, and 3 display ports.
