Help troubleshooting lots of random crashes with my GTX 780

Hopefully somebody on this forum can help.  I've been tearing my hair out over the stability problems I've been having with my 03G-P4-2781-RX since I got it. They've gotten so bad that I've had to remove the card for the time being just to be able to use my computer.

I've found that frequently when I'm playing games and sometimes when I'm not, I'll get a bunch of random lines or dots on the screen and eventually Windows will tell me that nvlddmkm had stopped responding and was restarted. Once this happens, it happens repeatedly until I restart the computer. Sometimes there is also a Windows blue screen.
 
I'm running Windows 8.1 and my motherboard is a MSI B85-G41 PC Mate(MS-7850) with a Intel Core i5-4690K processor.
 
Things I've tried already:
Upgrading to the most recent video drivers(340.52) and trying several older versions of the driver (337.88, 335.23, and 334.89) didn't seem to help at all.
Trying a different power supply. I originally had a Coolmax ZX-700 700W RTL but I also tried a Corsair CX750M but I had the same problems with both.
I've also run a temperature monitor program and my CPU and GPU temperatures don't ever seem to be above 70C when the problems start occurring.
 
I'm also not doing any kind of overclocking.  Anybody got any ideas on what the problem might be?

    Hello and welcome to the forum. From your description (artifacts on screen), it reads like you have a defective GPU. I'd contact EVGA directly and discuss a possible RMA for your GPU.

    it sound like a VGA ram issue to me, best to RMA the card.
