GTX770 Fans Stuck at 100% Speed

Hi all;
Search feature on forum doesn't seem to work at all, but I am  just curious if anyone has ever had this problem before.
 
Fans on card stuck running at 100%, permanently, starting as soon as PC powered on and continue into windows (normal + safe mode).
2GB GTX770 SC, w/ acx 2 cooler.
 
Already RMA'd the card but just curious to find out why it happened based on maybe someone else's similar experience or knowledge.
 
HW monitoring programs showed fan RPM at 0, and fan speed at 41% (even though they were obviously approaching lift-off speeds) & was not alterable at all through any programs (precision, afterburner, nvidia control panel, etc. which were only installed one at a time - after problem started - issue follows card to other pc's also)
 
Started intermittently running at 100% for no apparent reason, before it finally spun up to full speed and stayed there forever more, even on other pc's.

Again - not seeking a solution, card already RMA'd - just curious as to what the problem could've been for my knowledge/future reference.

    Card most likely had a faulty fan controller or the fan header wasn't seated fully.

    I wonder if this is something I could have attempted to fix myself or if RMA'ing it was really the best option.
    I say this because postage for RMA is $60.
     
    If something was faulty though I don't see that being within my control, something not seated fully though, that may be within my grasp...
