New 304.79 beta drivers may fix this down-clocking issue for some/all of you:
 

Fixes an issue where some manufacturer’s factory overclocked cards default to and run at lower clocks.

I literally just finished installing my new 680s about 15 minutes ago and ran a Heaven bench as a quick test. First time through, it kept dropping to 30 FPS every few seconds, so I exited out and checked my settings in Precision and the nVidia panel (I left Adaptive Power on). Re-ran the test, and no framerate drop this time. I'm using the newest beta drivers.
 
I'm really hoping this is just a fluke in the drivers or firmware and not a sign of a bigger problem. I was starting to get nervous that my motherboard's SpeedStep might have been at fault somehow.

The best way to tell if your GPU has the same problem that some of us have seen is to monitor the "Power, %".   My faulty GPU shows the power shoot up to 275% when the clock and framerate drop.

I've got the same issues. It's not so bad if I leave the settings at stock. If I do any overclocking at all it frames will drop from 60 to 30 all the time. I've heard that if you bump the power target all the way up it isn't supposed to drop. IDK how true that is. I'd like to know what's up. $630 on this card and I got better performance from my 560....

I'll start monitoring it as I play around with the cards. Like I said, it happened the first time I tested Heaven, but not the second, nor were there any problems with 3DMark 11, so who knows what's going on...

My new 2nd GTX 680 FTW+ is only running at 1163~1176 clock instead of its default 1189 clock. Sigh did I get a 2nd bad card again? Running newest Nvidia beta drivers and precision X. And yes my 1st card is still running fine at 1189 and no heat isn't an issue.

That's odd your guys' power jumps way up... when it has been happening to me with 690's, the power suddenly just drops and then clocks and framerates drop as well, and game often crashes. And yes, power is not an issue in this equation. I swear it's never ending problems for these cards.

You may be right I might have been mistaken. It looks like according to the EVGA website, the boost clock on a 4GB FTW+ is only advertised to be 1150mhz and the Classified is at 1176mhz. It was strange because my first two cards always defaulted to 1189mhz boost. I guess I just got lucky? I could SWEAR the FTW boost clocks were higher. Did Evga officially lower the specs to meet market demand I wonder? Well I guess I just got to see how far I can push this 1176mhz card. I guess I should be fortunate that's it's not just 1150mhz?

As you can see in the screenshot its happening at 45c which is silly. I'm reinstalling my OS so i can debunk this problem without any outside interference. If its happening on a brand new install, that tell me the card is gonna have to be RMA's :'( i hope not!

Ive got the same problem, Batman, skyrim ect.. these's cards are crap. Trying to play games @ 100 fps with no dips is inpossible. i drop to 60 fps and power at 50%..WHY.
 
mY 480S run the game at 100 load,
680 carnt run the game at rull load, Reformated, just downloading game's will report in new thaerd later.  sigh
 
Should have listen to my heart and not my head.
 
ps if my 480 add more ram i would have stuck with them, great cards... I can get them to load fully

Installed the new drivers and it makes no difference. They still do the same thing. Although it does seems to happen more often now? Ill post a screen shot of whats happening when the chaos with the card stops.

Temp wise they rarely even go over 50c so i dont think thats the problem. Im starting to think its the cards problem :(

 
When I run Diablo 3 or Starcraft II, according to SpeedFAN my gtx 680 ftw 4gb reaches a max temp of 74C.  All settings are at stock.

Try turning  the power target all the way up in precision. That should make it hold at the max power level you have setup. Seems to be working for me...

You were very lucky as already said the advertised boost is 1150. With both of my FTW 680's they boost to 1163 stock.

It is, its terrible. Im trying to test every possible solution, but at this point it just looks like a possible bad firmware or a bad voltage regulator? They want me to RMA, but want me to pay and ship the card to them? I think im just going to go through newegg and get the free shipping. :)

I have the same problem now with my EVGA GTX 680 ftw 4gb. It downclocks randomly from 1262 to 324 in BF3 and also in all benchmarks. It seems to fix itself after pc is on for at least 20 min. or If you run it through a full benchmark and then restart pc. It started doing it about a week after install. I'm guessing it's either NVIDIA driver or new EVGA Precision X ?

I installed both my cards and configured them for surround in SLI when I got them.   The very first thing I did was start up Heaven and I immediately saw the issue,  stuttering due to low framerate periodically.   However, in the process of trying to figure out what was wrong the issue seemed to "fix" itself after the PC had been up for a while.   I soon found that the problem returned after a reboot or waking from sleep(power off).
 
I don't think this is a software or driver issue.   I removed my cards and tried them one at a time in the same slot with the same power cables.   One card works flawlessly and the other exhibited the symptoms we have been discussing.  I tested both cards using the lastest released driver and the beta driver.  
 
There seem to be a fair number of cards with this issue.  I'm getting mine replaced, but it would still be nice to know what the issue is.   At least the issue seems to fade away after the PC has been on for a while.   However, it is very annoying to have the clocks jumping arround for the first half hour under load.

I see this happen randomly in modern warfare 3, but not in BF3...
Mw3 has drops to 25fps... I haven't tried raising the power %... The card doesn't full throttle like it does in BF3 which leads me to believe its a driver issue.

It was doing it constantly in mw3 until I system restored and forced steam to do an initial direct x setup... Now I see it happen intermittently

My guess would be you are having some sort of software conflict. Most likely between the MB software and Precision and/or anything else that polls the same sensors like HWmonitor/Speedfan Etc. And your continued use of the Precision OSD is most unwise, as there are known bug issues when using it....
As I posted before, you should attempt to -rule- out- such a conflict by creatively troubleshooting with one or both such softwares completely uninstalled.
 
At this point I guess you should just wait and see what the RMA card does. If it does the same thing, then you will KNOW its your config.

Its not precision.
 
i have ran the card with and without it ( installed/running) and the card continues to do the same exact thing. Sure i cant "watch" it do the same thing but i can see my frames jump in game from 15 to 60 rather easy with my own two eyes :)
 
Besides, i have ran this card in a completely different rig and it does the same thing.

I have the same issue with my gtx680 2GB. The GPU clock speeds remain constant at 324. This leads to an mind blowing 14 fps. Not sure what else to do and ive practically tried everything.

I would be interested to see if there is a resolution to this as well.  I believe this is tied in to the same issues I'm having with my 680 FTW+ - it randomly clocks all over the place, and it crashes at random times in benchmarks or in general use.  It's crashed once on me when I was just in the BIOS.
 
The card I have is displaying the same behavior in two totally separate machines, and another card (480gtx) works great in both under all conditions.  I see a lot of people on several forums that are having this issue with the 6 series, and it seems like some folks are just lucky and get good cards, and others don't.  I'm going to try for an RMA and see if that helps.
