    Could you re-seat the card and make sure the power cables are completely connected. Use a driver wiper program such as DDU to completely wipe all driver related files and FRESH re-install all the drivers. Reset all settings to stock including the settings in Precision X. Play a game that makes your crash and report back.

    Questions:
    Are the settings at stock or overclocked with Precision X?
    How long ago did you notice the problem begin?

    CPU: FX-8350 Black Edition | Ram: Corsair Vengeance Pro 8GB (2x4GB) @ 1866 9-10-9-27 1.5v | Mobo: Asus Sabertooth 990fx R2.0 | GPU: EVGA GTX Titan X | PSU: Corsair AX760 Platinum Rated | SSD: OCZ Vertex 460 120GB | HDD: WD Black 1TB | Case: Corsair 450D | OS: Windows 8.1 64 bit OEM

    Probably your card work on fabric clock as last stable clock.
    It's little weird thing that usually benchmark tests pass on 50-100MHz higher clock than game stable...
    But if 1020MHz clock is last stable clock at least after 10-12 months card will become unstable.
    I listen Jacob when he talk with some guy in studio about fabric clock their cards and how much they OC usully, he say they usually go with some clock enough to offer good even best performance on market and to leave space for customer to overclock little because they must be sure to card work whole 3 years on same clock. For that I think usually need space of at least 50-75MHz for OC... But from other side he say they can't go too much with OC because no enough quality cheap. That is true offer would be very small if they launch GTX780Ti with 1100MHz based clock example. 10% cards could resist 3 years gaming on such speed with all NVIDIA limitation and voltage restriction.
    Many people who OC their reference cards on 1300MHz boost they couldn't play all games properly even on GTX780Ti Classified boost 1150-1170MHz. Because of that is weird how card stop in benchmark and in game on same clock. In that situation maybe some other part on graphic card not work as should and have nothing with chip quality.
    But if card work on some clock as last stable even if pass benchmark test only on 20MHz more he will show instability sooner or later.
    No any doubt in that.
    Biggest chance when she need to work on 99% to allow gaming example  50-55% fps.

    Which Ti Classified is it? 2888 or 2887 for the part number.

    Have you updated either of the BIOS options in the past?

    You may be getting PSU issues, as this card does draw a little more than the 780 cards, but hopefully it is something much more simply and cheaper to fix.

    Like stated above, test the system to see if you can get it to crash.  Try reverting back to Precision 4.2.1 and see if it still occurs.  There have been a few instability issues with Precision x16, so it is more than likely tied to Precision.  Give that a try and see what happens.

    Given that you have only a single card and an AX1200, as well as the rest of your system, it's pretty close to impossible for that power supply to not have sufficient power for your system.  On the other hand, if the PSU is defective, then it could cause crashes for that reason.  If you go into your BIOS, and go into the Monitoring section in Advanced mode, what are your 3.3v., 5v., and 12v.?

    What game or games does this occur?  Have you tried going back to a previous driver?  You mentioned the psu was about 2 years old, but how old are the rest of the components, such as the video card, motherboard, etc.?

    Is the motherboard BIOS up-to-date?  The rest of your drivers up-to-date?  Not just the video card, but also chipset, Audio, network, etc.

    I have the latest BIOS for the MB, Asus X79 Deluxe I installed it late 2013. I have updated all the drivers. I tried to run 344.11 with precision 4.2.1 and still the same issue my voltages are:
    CPU 1.008
    3.3=3.296
    5=5.000
    12=12.096
    I have tried re-installing direct x and Visual C++, I haven't loaded any revised BIOS on the card and just bought it new from new egg last month, I am running it on the Ln2 bios it happens no matter which bios is used. It doesnt happen if run at stock at all, anything over 100 mhz boom total driver crash with message saying driver hung or driver has stopped working.
    It happens in COD ghosts
    Battlefield 4
    COD black ops 2
    COD 3
    I have reinstalled the driver after deleting with DDU and will report back

    you should try using Classified voltage tool =to very slowly bump the voltage.  Go to overclock.net and scroll through the first post if you WANT to slightly overvolt your card.  Push +100 mhz is a LOT of difference compared to what you mention "anything other than completely default settings."  Sorry, but that is the same thing I get with my card.  You will need to start giving it voltage as well.

    If you aren't overvolting with the classified voltage tool, then it may not help you overclock.

    Been overvolting thru Precision X 16 usually a little over 1.2, I will try the tool.

    I have been testing the voltage change in x16 occasionally, and it works perfectly for the voltage, but once voltage is applied the crashes still happen, so I think there is a conflict within the software.  Using the Voltage tool seems to break the conflict, so I am not entirely sure what causes it..

    The card is fine if you're not crashing at default speeds. Anything above default speeds isn't guaranteed.

    Yeah I would agree but now it happens with no OC in COD ghosts. I am going back to step 1 in safe mode now and am searching forums for completely erasing all former installs. I have uninstalled precision x 16 and the voltage tools. I will probably go back and turn PCIE spectrum back on just to start at the beginning. I will try without precision x being installed. It happens in exactly the same exact spot. I tried redownloading and verifying game cache but had no effect. i also reduced the quality setting but that didnt help either.

    I have had some success so far by uninstalling precision X completely and uninstalling all drivers and reloading each one after thoroughly cleaning the registry. I started with 337.88 and have just installed 344.60 and play a while. I haven't tried any others right now want to see if I stablize just ghosts right now. It actually started with BF4 with a DXGI_ERROR_DEVICE_HUNG_ERROR, since installing 344.75. Yesterday it changed to the Driver has stopped working error. These are all stock settings I do not have any precision installed right now, I may try to re-install the earlier version if I can successfully work my way into 344.75.......

    DXGI_ERROR_DEVICE_HUNG_ERROR

    This error usually happens when precision gets corrupted or your card is unstable. If the error doesn't show up after removing precision then that was most likely your issue.

    I am running very well with 344.48 and precision 4.2.1 and +50 mhz so far, 344.60 huge artifact problem in ghosts. No problems now with ghosts or BF4 so far running this driver. Could this be a driver error. It seems to be known issue. Think it is safe to use X16 or just stick with what works?

    DXGI_ERROR_DEVICE_HUNG_ERROR is usually unstable card same as Driver Stop Working and Recover...
    If you found stable point don't change nothing. But I afraid if card is unstable on default settings crash will back.

    That's shame for EVGA, underclocking Classified cards.
    If that's last solution, that would be definitely last EVGA graphic in my life.
    Why people pay 150$ more, to go in opposite direction, instead up with clock, they should go down.
    Someone buy cards with higher clock to avoid overclocking, now must to do that only in opposite direction.
    More graphics like that and ASUS will sell more motherboards and graphic cards to ex EVGA customers.
    For them is fun to read to such problems on their site because that nicely explain why their clock is inferior and turn customers to them.
    Anyway stance of most people there is that EVGA motherboards are inferior from software and hardware side to ASUS motherboards with far less options and much worse compatibility with RAM, HDD, SDD, M.2 devices and more unstable graphic cards than anybody because higher clock and that customer pay price of that at the end.
    EVGA lose concept completely, one series show up, people hurry to buy cards and they are usually collateral damage because after 3 months show improved version, or never know, bad calculation in supplies and after you open box you recognize completely other memory, VRM, no EVBot etc like nothing happen.

    Why EVGA customers need to install Precision X if don't want to OC and software is full of bugs?
    Question for 1.000.000$. To downclock EVGA hardware because stability in games.
    OK we must be fair, they are not made for games, only for numbers and futuremark and only in weightlessness condition as in Orbit.

    Before 2 days I look video where guy nicely explain difference and improvements on EVGA GTX970 FTW + compare to EVGA GTX970 FTW.
    Maybe some idiots without emotions and think on hardware... but first what I thought I is how feel someone who pay same price in first weeks only to get "first" GTX970 FTW and I stop to look video, I couldn't listen any more. Because such thinks companies will sink deeper and deeper.

    Since I am the one that started this thread....I ended up RMAing that particular card and getting a replacement card from EVGA. At the risk of getting flamed....I have given up on the notion of trying to OC at all since the most I could get out of the second card with a skynet bios was +35 for everyday use. I have an ASUS x79 deluxe board and a 4930K chip at 4.3 Ghz. I guess there are just some cards that do not clock over well and a least the last few I have had this has been the case: EVGA 770 ACX, 780 ref, 2 780 ti dual classified. I am runnig well now at stock and really better without any crashes since just running on the stock LN2 bios.....so much for my update, i never had to downclock though

    At the end that's most important if you can play on fabric Normal or LN2 BIOS with Boost Enabled without crashes.
