GTX 680 FTW+: Random spikes to 276%+ TDP

I have a really odd problem that I can't seem to find anyone else having.  Many times whenever I first start up a game my graphics card will throttle over and over again to minimum clock for 5-15 minutes then it will settle and never do it again until I start the game back up at a later time.  I opened GPU-Z to watch what was happening and noticed the TDP was spiking to 277% over and over again which obviously caused the throttle.
 
I've tried fully uninstalling and reinstalling the nvidia drivers to no avail.  Anyone have any idea what's going on?  Has there been cases of faulty power consumption sensors?

    Wow, i can't believe my googling didn't find anything.
    Cards pretty old, hope I can still get an RMA at this point.  I bought the card from a friend (who bought it off amazon im guessing).  Trying to get invoice from him now.
