  1. Which USB devices and/or ports are failing? All ports, Anything that I plug in iphone, Mouse external HDD
  2. Which motherboard and BIOS revision are you using? E770, Bios 77
  3. Did this problem occur right away or over time? If so how long did it take before the problem showed? Does this issue occur on every system boot? came over time I've had my board over a year
  4. Does this issue occur outside of the Operating System? Such as in BIOS? If not, which Operating System is being used? Yes
  5. Has the motherboard been RMA'd by EVGA? If so did the issue go away or persist? No_This is a replacement to my original e760

Add me to the list.  The board now randomly disconnects the USB mouse.   It's not software.  The archive from the new install does exactly the same thing.  
 
EVGA RMA'ed the board.  The replacement has the same problem, just more frequently.  The system has the keyboard and a microsoft USB wheel mouse with the lastest drivers.  The windows add/remove hardware sound prompts often.  The light in the base of the optical mouse goes out and stays out until it's unplugged and replugged.  It doesn't much matter which port it's plugged into.  With a mouse plugged into  two ports, two mice fail at random.

I'm still experiencing USB drop-outs. The really weird part of this is it seems to happen in spurts. My rig will go for a week or two without a drop out, and then a whole string of them. I wish that EVGA would keep us posted or something. This is getting old now.

Complete drop out. I have a Logitech G19 and a Logitech G9 running through it's USB 2 port. Of course both die.
 
2. Which USB devices and/or ports are failing?
Not real sure. Sometimes all will stop sometimes I just switch the usb's.
 
3. Which motherboard and BIOS revision are you using?
EVGA X58 X3 (E758-A1) Bios rev. 82
 
4. Did this problem occur right away or over time? If so how long did it take before the problem showed? Does this issue occur on every system boot?
This problem occurred over time. I've had the motherboard since Nov. 2009. And, it was very sporadic then. It just started to happening with more frequency since Jan. 2011.

 
5. Does this issue occur outside of the Operating System? Such as in BIOS? If not, which Operating System is being used?
It hasn't in the past. But, just today it started happening again. I rebooted, and switched usb ports and the bios said there was no keyboard present. Switch back to it's original port. And, bingo...it works.  I'm using Windows 7 Home Premium X64

 
6. Has the motherboard been RMA'd by EVGA? If so did the issue go away or persist?

  2. Which USB devices and/or ports are failing? all ports and every every device i won stops working. usb flash, kb/mouse receiver. heck i got crazy one night and even tried USB internet.
  3. Which motherboard and BIOS revision are you using? e758 bios 77 i think. my original board i used pretty much every bios when it was released and  the problem still occured.
  4. Did this problem occur right away or over time? If so how long did it take before the problem showed? Does this issue occur on every system boot? over time the problem started happening. it acts like its degrading over time. get worse and more frequent.
  5. Does this issue occur outside of the Operating System? Such as in BIOS? If not, which Operating System is being used? yes it happens outside the OS. in bios nothing will work unless i shutdown the PSU and let it sit unpowered for 5 minutes than it will work again, for awhile.
  6. Has the motherboard been RMA'd by EVGA? If so did the issue go away or persist?  This is happening on my original board and the RMA replacement i have received in the last month that is being RMA'd for sound problems..  one board after another is broken.
 
What i notice is that the problem persists across amny different types of boards. there is pretty much one common denominator.  I wont name names becasue the fanboy's will come running.
 
Is it an issue with the chipset? is it a board manufacturing problem?

I think it is either a MS hotfix issue or a chipset issue (Intel).

Upon further investigation into randomness of my USB drop outs.  I have found there could be (most likely is) another problem lurking here that goes unnoticed.  And, has absolutely nothing to do with USB drop outs.
 
I read on Logitech forums that the G19 (and most likely all of the G-series keyboards), are sensitive to ESD or EMI emissions from GSM phones.

As fate would have it, I was reading through what CharlesB posted and at that moment my phone received a email.  My keyboard just shut down and rebooted.  I have since moved my phone away from the keyboard and it has yet to drop out. 
 
If your experiencing similar random drop outs.  Try moving your phone away from the your keyboard.
 
I will of course monitor this, and post back if it drops out again without the phone in close proximity.

i just got snakebit by the USB thing.
 
never was right from the start.  keyboard (logic tech G510) loading is/was hit and miss.
i would just plug into another USB slot ,  but now none of them is working.  happened over a 2 day period.  i am just getting everything rolling ,  no phones , no games i was just loading programs . only things hooked up to USB,  keyboard and mouse
 
 
just moved my phone away from the keyboard and presto i have action again.  maybe thats it.  ill keep you posted.

spoke too soon ,   i got 1 reboot the nothing again.   i now have 0 USB slots working.

I dont mean to insult Krec1.  Did you install the mobo drivers.  There really is a usb problem with some of the x58 motherboards.  Just making sure.

i just bought a nice logic tech g510 for 90.00 it is now on my 350.00 emachine and my 19.99 ten year old keyboard is on my 3500.00 rig .  both are working like champs now. i guess thats how i must roll.  i can work on OCing while this usb thingy gets worked out.

Ok so I do not have an X58 Board I have the 790i But I am having the EXACT Same issue as everyone else that I have read through here. It really looks like being able to Disable EHCI / Legacy USB MAY Fix the issue although there have been some reports that issue does come back. I have had all the issues and a little more and have tried many things but I am 99% convinced it is not the hardware but something to do with USB Support.
 
I have tried the Logitech products on my laptop and have not had any issues they only appear to have issues when using this machine / board. The Logitech forums know about the issue and it appears to be a real popular one but not being able to disable the EHCI there is no way to test to see if it resolves the issue.

  • USB would Lag and Keep typing letter and then eventually Freeze. Pulling USB and re plugging appeared to resolve this until it came back minutes to hours later.

  • Actually this happened out of the box however, due to the software it was happening in I thought it was just a result of it (being in Beta and all). Today is the first time it happened outside of that Software which started my search and led me here.

  • Have not used it outside the OS to know.

    Still having this issue. About 1 out of 10 times I plug in my iphone to sync/charge at night I will suddenly lose all my USB devices. Keyboard and mouse lights flicker for a sec then go dim.....
     
    Only way to fix is hard reboot....

    On boot Logitech G15 keyboard (also happens with the G510) keys will not work. Cannot get into BIOS. Also will not correctly wake up from S3 sleep. It the PC powers on, powers off and boots normally rather then wake from S3. Additionally after I installed new nVidia Geforce drivers, on reboot the keyboard would not work during login screen. Normally the keyboard only does not work during BIOS and post, and OS (F8) menu selection.

    Any USB port the keyboard is plugged into seems to behave this way.

    I believe the Logitech Keyboard worked correctly under a different older version of the BIOS. I only recently (Feb. 2012) have upgraded to the E75883 release.

    Occurs during POST, and recently occurred in Windows 7 login screen, where num lock would work. But other keys, such as ctrl-alt-del would not. Unplugging keyboard and moving to another port did not seem to cause USB re-detection to occur.[
     
    No, the motherboard has not been RMA'ed.

    Funny, I've been getting this recently. Not the first time, i'm sure it won't be the last. Headset dies out, KB dies out, and mouse, pretty much only those three sub devices. Nothing else. Oddly they are all Logitech :P Also don't matter if they are rear USB ports or front ones. Annoying as hell, and if I had the money I would RMA, last board I had before the fry did not act in this manner.

    A few months ago my computer would fail to post once in a while, and I would have to hit the power button a few times to get it to go.  Recently my Zboard Merc keyboard would time out on one of the backside ports everytime I would stop using it in game (afk).  I then noticed my USB network adapter was cutting out my internet every so often.  Finally in an effort to isolate the problem I did a clean Windows install along with a fresh BIOS Flash and new drivers.  The problem still persisted.  The solution was to raise the ICH voltage to 1.2v, when raising it to 1.15v, like what was recommended, I still had the problem.  I notice my system also seems a lot zippier now, and my network adapter does not cut out any longer.  For a board that was bought in 09 it has been my most reliable mobo yet.  It has been running an I7 920 at 3.4GHZ around the clock for 3 years now.

    I came to this thread because I was having odd USB problems with my X58 SLI, but after some process of elimination, I determined it was caused by the Acronis Nonstop Backup service.  I set it to disabled (I don't use the non-stop backup feature) in services.
     
    My problem was that whenever I would plug in a thumb drive, it would take about 30 seconds for it to appear in Windows Explorer, and disk errors would be logged in the event logs.
     
    I consider myself lucky now!  I've not had the dying USB ports problem.

    My USB ports recently failed but I've managed to get some older USB hardware working but the ports are not 100% functional...

      - A few months ago all USB ports failed and all USB devices stopped working. A reboot confirmed that all USB devices could not be recognised. I had to resort to using a PS/2 keyboard to get by. I was able to update the BIOS and install 15.57 nforce chipset and network driver update. I tried deleting the USB host controllers and rebooting to reinstall the drivers. I also tried a hard clearing of my mobo by unplugging my PSU and removing the mobo battery.
    2. Which USB devices and/or ports are failing? - All USB ports seem to respond the same way. Current devices that are working are MS Wired Keyboard 600 and MS Optical Trackball 1.0 and a generic 1 GB USB key. Devices that do not work are any wireless keyboard or mouse and wired Gigabyte Force K3 keyboard.
    3. Which motherboard and BIOS revision are you using?
      - 122-YW-E173 - nForce 750i SLI with SW20 BIOS.
    4. Did this problem occur right away or over time? If so how long did it take before the problem showed? Does this issue occur on every system boot? - The first failure happened right away and completely stopped all devices from working. I was able to "revive" certain devices but not all of them regardless of how many times I rebooted. At this time, only a few "older/simple" devices seem to work. Newer USB devices are not recognised.
    5. Does this issue occur outside of the Operating System? Such as in BIOS? If not, which Operating System is being used? - Yes problem still persists outside of Windows 7 - 64 bit.
    6. Has the motherboard been RMA'd by EVGA? If so did the issue go away or persist?  - No, this my work computer so I don't want to RMA unless it's a last resort.
     Please list all usb devices you have installed.  - MS Wired Keyboard 600 and MS Optical Trackball 1.0.

    b4 i knoticed this topic....i had trouble with usb ports ever since A74 came out.

    had trouble with all usb ports with newer bios (A74)  for p55 ftw,,, using razer lachesis ..broken 2 mice for this reason lol .. so i download all the bios n tested which one got rid of my isse and a51 been helping .. so far no lagging or slow downs with my mouse....  mayb you shouldnt touch the bio n get new programer who can write the bios .. just idea lol
     

    i went back to this bios so far stuff works  
     
    A51 Updates:
     
    * Improves USB Compatibility
    * New CPU Support
    * Improves Memory Compatibility
    * ClockGen Switch Mode Disable Option to not power up twice
     
    A70 works great too.. 
     

    ever since i totally got rid of a74 bios everything seems to fuction better for me now ... as weird this going to sound,,,  but when A74 bios was install on 3 on bios sel. all my other bios where affect by a74 bios they got same usb issues... i got rid of A74 switch to A51 everything works again no usb issues. mayb just me i dont know lol

    quick ? why idiots at evga grey out usb option (bios A74) that you can choose highspeed or fullspeed options ...in every other bios you can pick w/e option and not grey out...
     
    can this b cause or lag issues ... with usb ports ...

    Not too long after boot, though not immediately, I get an error from Windows this USB port has stopped working (Code 43).
    1. Which USB devices and/or ports are failing? Port 5, Hub 1
    2. Which motherboard and BIOS revision are you using? 680SLI Mb & NF68_P33 BIOS
    3. Did this problem occur right away or over time? If so how long did it take before the problem showed? Does this issue occur on every system boot? Just showed up out of the blue after having the PC built up for 3+ years.
    4. Does this issue occur outside of the Operating System? Such as in BIOS? If not, which Operating System is being used? I can't tell. OS= Win7 64bit
    5. Has the motherboard been RMA'd by EVGA? No
    What exactly is failing? Is it a hardware failure on the USB support on the motherboard? Only one port is failing, all others are still working. How do I fix this?

    I have a single device that keeps turning itself in Windows (Code 43 in Device manager) off b/c of a 'problem' on my e770 mobo - should I be reporting it here with the necessary info?
