So in the last couple days.. this has been happening.  The screen is not bad.  I have to unplug the displayport connector and then the middle screen (hdmi with dvi adapter).  Would this be a driver issue... or is this what i hope it isnt, artifacting.  I run nvidia surround with 3 screens. Dell 24".

The middle screen has a problem and it's the one with the adapter?

I'd wager that it is the adapter (or maybe the cable) going bad. Do you have a different cable to try out with it? A different adapter?

All the cables are new Hdmi that came with the dells they are like 4 months old.  Its a new dvi to hdmi so idk, I unpluged it and used it with a different screen cable and i didnt get the same issue.  I then switch the cable to a different screen and both cables works fine.  It only does it on boot up ever so often.

I'm wondering the monitor frequency is being exceeded by control panel when it happens?

They are 60hz 1080 screens.  About 2 months ago i did a driver sweep due to another issue i had and did a clean install of the display drivers.  It only does it on startup here and there, and when i replug the cables screen works fine.  Any game i play i use vsync and never have any issues.  Its done it twice this week now, which is the most its done ever.  My tower has excellent cooling, the 980 ti Classified never gets hot, like never goes over 50-60 C max temps, plus i dont overclock it, its all factory settings.  Other thing is it doesn't do it on the splash screen for the motherboard when it starts up, everything is normal, only when I get into windows.

If I'm reading what you wrote correctly (I've bolded & italicized it in the quote), you've identified the adapter as the problem. Time for a new adapter.

Sorry I didn't write that well.  I unplugged all the cables, used all with the same adapter, and all the screens work.  If, it is the adapter, would it be best to ditch the dvi>hdmi, and get another displayport>hdmi?

Dp and hdmi are more similar. Dvi is a whole different breed. I would say go for the dp hdmi adapter instead.

If I was having the problem I'd do it.
