eVGA GTX 760 ACX dying?

Yesterday during a Diablo 3 play session, noticied some random artifacts in the screen, with several nvidia display driver recovery by the OS and game crashed with a D3D error.
 
Recorded a video of this problem (but cannot post links).
 
How the test to confirm its the GPU is dying?
 
note: never overclocked this VGA and cannot removed the cooler apart of some superficial cleaning. The case have good air flow (3 outtake 120mm fans)
Config
- ASUS Maximus V Gene
- Core i5 3570k
- 16 GB DDR3 2133
- eVGA GTX 760 ACX 2GB
- corsair h100 (cpu cooler)
- corsair case micro-ATX
- SSD samsung 128gb
- Antec true power 750w.
- Windows 10 pro 64 bits
 
meanwhile, this graphics card have a unrelated defect in the corner of PCB (the corner started to broken whith six month after purchase, now have 2 years and six months), but is concern about RMA refusing? (never bumped the card against something not other components inside the case).

    Hello,
     
    There could be some issues with your current or past driver installation. I would completely wipe the drivers with a program called DDU (display driver uninstaller) which you can download here:

    If you ever had an AMD card in your system I would also use the tool to wipe the AMD drivers as well. After running the program and removing all your current display drivers re-install the graphics drivers and see if the issue persists.

    Ok, i trying to clean the drivers.
     
    but i downloaded furmark, with fan at 100% and crashed after some seconds. Temperatures reached 71 graus.
     
    I installed a AMD 4770 to verify if this problems persists.

    Have performed a cleaning drivers and reinstalling and cleaning the card the problems persist.
     
    With AMD 4770 all runs fine.

    If you haven't yet I would recommend contacting our EU support at supporteu@evga.com, or by submitting a ticket under the card once it's registered. It does sound like the card may be malfunctioning.

    What is the part number of your video card? Do you notice the artifacts in any other games?

    Those definitely look like artifacts to me, although I would like a screenshot to be sure. If what you're seeing on the screen doesn't show up on the screenshot, it may not be the card at fault. @MhzManiac, you need to replace the spaces with periods. It showed up fine for me.
