Bad motherboard, or bad 980?

I have a brand new PSU and a replacement card (I rma'd my 980) and I am still having an issue.
EVGA SuperNOVA 750 G1 Power Supply
EVGA GeForce GTX 980 Superclocked ACX 2.0



My drivers restart for 3 seconds, while this happens both monitors restart and you can hear the game audio continue in the background while the game keeps playing. Once they restart everything goes back to normal unless the monitor just stays a solid color like in the pictures, this requires a restart because you cannot interact with the game anymore.
Could this be a faulty motherboard, I am literally replacing all of my parts...

    Sorry but I am not going back to windows 10 unless I am forced to because a specific game requires windows 10. The issue occurs on a single and dual monitor setup, I have troubleshooted with EVGA, ASUS, and microsoft, was looking for actual help like someone who knows why the error occurs or what could be causing it, not shots in the dark like update your video drivers or reinstall windows.

    I have a brand new PSU and a replacement card (I rma'd my 980) and I am still having an issue.
    EVGA SuperNOVA 750 G1 Power SupplyEVGA GeForce GTX 980 Superclocked ACX 2.0



    My drivers restart for 3 seconds, while this happens both monitors restart and you can hear the game audio continue in the background while the game keeps playing. Once they restart everything goes back to normal unless the monitor just stays a solid color like in the pictures, this requires a restart because you cannot interact with the game anymore.
    Could this be a faulty motherboard, I am literally replacing all of my parts...
     



    Some more basic information please:
     
    System specs: brand & part number - MB, CPU, RAM, type of cooling, additional cards installed, monitors & kind of cable used - more is better
     
    Basics: 
    Are you running the latest BIOS on the MB?
     
    Are you OC (over clocking) the system?
     
    Are your Audio drivers up to date (cause of many issues)
     
    Does it only happen in one game?  Name of game(s)
     
    Does it only happen on the same monitor?  If yes, I'd switch the monitors and see if the issue follows the monitor & cable combination.

    Regarding your question "Could this be a faulty motherboard" - that is a possibility.
     
    What has changed?  New game?  Always happened - Just started happening?
     
    You have up to date security software - scan comes up clean?

    P8Z77-V lk ~Not running the most recent version of BIOS, I sadly cannot update the BIOS because I don't have a USB drive- I ordered a thumb drive but it hasn't shipped yet.

    Happens in Path of exile, counter strike global offensive, league of legends, team fortress 2, fallout4.
    Happens on both monitors, the game is always on the 1080p monitor so usually on that one, but sometimes the left monitor simply crashes to a colored screen and a restart is required for it to be responsive again. I have two secondary monitors available but only one main monitor. It happens on both secondary monitors they're just throwaway VGA dells.

    I actually spoke with a microsoft employee and he specifically said a lot of games malfunction with windows 10 and if you are one of the people who have issues it's more safe to stick with what works, windows 7. I'll take his word for it.

    You will need USB 2.0 drive and format fat32 for flashing BIOS.  Lost of MB refuse to flash with USB 3.0 drives




    My 3.0 USB slots don't work without that driver, my status on amazon is "ShippedNov 25" so I cannot flash the BIOS even if I wanted to. I will get rid of the driver though doesn't matter to me, worst thing for me is flashing the BIOS and it doesn't fix anything.

    I am using build 10586 and outside of disk check games, everything works fine so far and I own hundreds of games

    You will want to look at Min & Max on Volts & Temps.
     
    Confirm in Windows device manager that all is well - NO yellow circles or Red X warnings
     
    Update ALL MB and Audio drivers.

    curious as I installed Windows on my box with a USB stick and USB 3.0 on the front and it was not a problem for any SKU
     
    I use a tool called Rufus to make a UEFI stick for my tablet, as the Windows 8 USB DVD tool does not support UEFI for 32-bit

    Their is circuitry to filter the power output & a battery or capacitor to keep the power output constant.

    I have seen issues when people run their laser printer - cause large power drop on same circuit the PC was on and cause stability issues.
     
    Just one example of why UPS is a good thing to use as the power to/in your house is not always stable.

    Not just power interruptions - spikes - sags - brown outs - noise


    They all can cause issues. 
     
    The link was for a larger one like I have - you could use a smaller cheaper one.
     
    Your 120V outlet  - probably is not as stable as one would think. You would need an oscilloscope to see noise and variation.  But hey if the lights don't flicker you PSU may be enough. 
     
    UPS just takes one more variable out of the loop.

    Windows device manager seems fine, no marks.
     
    Can't update MB drivers atm.
     
    I don't know what a UPS is, is that an extension cord with surge protection?

    CPU Vcore Min is on the lower side; your other V look good.
     
    Clear the Min & Max, run it in the background while gaming see what you can record when it crashes.
     
    What are the CPU temps - you can collapse some of the info like fans, etc.

    Crashed, couldn't interact with HWmonitor. Played like 10 minutes of a deathmatch.

    I think you'll need to enable 'logging' in HWMonitor and save the results to a log file so you can examine the results after the crash.
