I just happen to pull up GPU-Z while playing and a ton of pink (Thrm) came across the perfcap.
 





So it is throttling because it is hitting temperature cap.  It is showing 81°C and you can see it just dropped so it was probably at 83°C when it happened.  Did you set a custom fan profile?  It only shows 49% fan speed.  You are going to have to raise that.

Run the same test and turn your fan to 100% (yes, it'll be loud) so you can check the results.

Right now i am running +200 on both memory and GPU Clocks! You need to OC in increments, do not jump to my OC speeds when you first run your OC's!! Better to go in increments of 5 or 10 to get started, and test with EVGA OC Scanner or Furmark. If you have no artifacts after 30 mins -1 hour you are good to push it a lil further each time.....DO NOT TOUCH THE VOLTAGE UNLESS YOU HAVE A MULTI METER AND KNOW HOW IT APPLIES TO THE GPU SPECS!!!!! Please leave this at 100% and you can run it at 110%  and still be safe but I have learned with my Titan X this will crash games for me even with my OC. I can run all my games at this setting so far, but I also have a n AC that keeps my room at a chill 66 degrees and have my PC next to the cooler. I have no problems on my setup so far. :)

I have been monitoring the PrefCap the past couple of days and I've only seen VRel, not Thrm.  Should I still turn up the fan to 100% for VRel?

Thanks for the tip...noted to not touch voltage!  Once I get this VRel issue figured out I will start to OC.  I'm not sure why the card is struggling to run GW2 on my Crossover 43" 4K monitor.  I'm getting between 5-20 FPS.

Here is an update to where I am today.
 
I'm still having severe issues and I don't know what the hell is going on! lol
 
I contacted EVGA support again last week.  As usual, they have been very helpful and provided great service. The tech went over some different checks with me.  He mentioned I should not be receiving this low of Frames Per Second.  He went ahead and put in an RMA and sent me a new card.
 
I received the card yesterday and I am still having the same issues.  I just spent hours trying different things and taking pictures to explain.
 
Below is the breakout I just did:
 
The following was performed on my 4K monitor with Windows 10.
1. I uninstalled and reinstalled the drivers, plus the new GTX Titan X.  I booted GW2 to test out the new card.  FPS were still the same; 2-20.

 
 
2. I installed my original Titan X and used SLI.  I booted GW2 to test out the new card.  FPS were still the same; 2-20.  I also monitored GPU-Z and I had multiple PrefCap issues.







 
 
 
3. I ran 3DMark Fire Strike (free version), and my score was only in 11K with SLI.  Without SLI it's around 10K.
 
The following was performed on my 27" Del U2711 1440P monitor with Windows 10.
1. I uninstalled and reinstalled the drivers, plus the new GTX Titan X.  I booted GW2 to test out the new card.  FPS were still the same; 2-20.

 
The following was performed on my 27" Del U2711 1440P monitor with Windows 8.
1. I installed a bran new SSD and installed Windows 8.
2. Installed the latest drivers
3. Installed Geforce Experience and the latest Windows 8 driver.
4. I booted GW2 to test out the new card.  FPS were 10x better; 50's - 60's.

 
The following was performed on my 4K monitor with Windows 8.
1. Same as above.  I booted GW2 to test out the new card.  FPS were 10x better; 40-50's.


 
 
I am completely baffaled and have no idea what is going on.  Is my i7 3770K stock unable to handle GW2 or is it Windows 10.  I have a hard time believing it is either of them.  My buddy I play with has the next Gen I7 following mine and runs SLI 980 with Windows 10 and has NO Fps issues.
 
On a side that, something else has completely blown my mind.  On my brand new crucial SSD with Windows 8 Just installed, there are 3 different OS's to choose from!  2x Windows 8 and Windows 10.  Not sure where it came from, but it's a brand new drive with a fresh OS install.

 
If anyone could provide me with some assistance I would really appreciate it.

EDIT: Just to explain the last paragraph better. I performed a fresh install of Windows 8 from my USB drive. I made sure, there was only one partition and the drive was formatted. After the fresh install before I could access the desktop, I was present with 3 OS choices to choose from, 2x Windows 8, and 1x Windows 10. Just for the heck of it, I chose Windows 10, and I received an error; same with choosing Windows 8 that did not say Volume 1. Each time I restart or shutdown and turn on the PC, I have 3 OS choices to choose from. I have no idea how to bypass this or set one as default to not show me the choices!

For one thing, you have FXAA set on the windows 8 screenshots with the higher FPS.  You have SMAA High set on the Windows 10 screenshots.  You also have the LOD Distance and Shadows set to ultra on the first and only high on the second.
 
You need to perform the tests with the same settings to have a valid comparison.
 
EDIT: I also notice a few other settings differ as well such as high res character textures and ambient occlusion.

As far as the drive, if you didn't reformat the partition it still has the kernel for the other OS.  It eliminates and overwrites that kernel when you do an upgrade to the OS but not on a fresh install.

Ugh!!! GeForce experience must have optimized the game when I clicked launch. I normally launch it from the desktop shortcut. I had GeForce experience open to show both cards were installed and figured I would launch the game through it. I didn't even think that the settings were going to change! I will have to redo the comparison when I get home.

Well, I seemed to have fixed the FPS issue!  I reformatted my HD again and this time performed a fresh installed of Windows 10 via USB.  In combination of the fresh install and the new Gcard, I am seeing normal FPS (50+ for 4K). 
 
However, I am still experiencing PerfCap issues, mostly VRel and Thrm.  Here are some of the latest GPU-Z Screenshots over the past couple of days.

It' very nice because on EVGA Precision X you can see name of their graphic card models nicely.

I think Vlada was just making a casual comment.  He is a bit philosophical at times..
 
As far as the performance cap, you are mostly always going to have one reason or another.  You hit your temperature ceiling or relative voltage or max voltage.  That is pretty normal.  You can play around with a different cooling setup or changing your fan curve to be more aggressive or your case fans to alleviate the temp cap.

As far as the performance cap, you are mostly always going to have one reason or another.  You hit your temperature ceiling or relative voltage or max voltage.  That is pretty normal.  You can play around with a different cooling setup or changing your fan curve to be more aggressive or your case fans to alleviate the temp cap.

Alright, it looks like I will have to play around with the fans, but may just have to live with it. :(
 
Actually, I installed a new SSD and created a new partition.  I did eliminate the multiple OS issue, but it was not related to the reformat/new SSD.  It's actually an option within Windows that is tied to your account.  
 
Right click on 'This PC' and select Properties > Click on 'Advance system settings' > Click on 'Settings' under Startup and Recovery > uncheck 'Time to display list of operating systems'.  Here you can also select the Default operating system and other options.  I'm not sure how the 'Time to display list of operating systems' option became checked, but after I unchecked it my PC booted to Windows normally.

I don't have the thermal problem anymore after adding the EVGA AIO kit.  I actually don't even hit 50° anymore.  All that becomes a matter of preference, though.

Very nice, thanks for the info!  This will allow me to have a closed loop watercooler.  It should definitely help with the heating issues!
