Didn't see this post till now. I was definitely going to say the PSU. It's been a long time since i've seen a Ultra PSU still in use. When using an older PSU it's not that it's bad, it's just not compatible with the new hardware. A while back I had a PC Power & Cooling 1Kw PSU and there was a revision of it that allowed it to work with newer hardware. I sent it in for an RMA and I got a revised PSU back with no issues.
