Is this GTX 970 capable of driving two screens at 4096x2160?

I have two LG 31MU97-B 31" screens. They run at 4096x2160. Can this video card drive both of them simultaneously?
 
Also, what if I only connected one of them when I play games? What kind of performance can I expect for, say, GTA V graphic quality?

    Yes, you can run two monitors at that resolution. However the lack of two Display Port might be an issue. You're going to need to use a DVI-D to Display port adapter as you're limited in FPS using HDMI. Your monitor's information page from LG...
     

    If you're willing to reduce settings to medium/high you can probably manage 30 FPS on GTA 5.

    I'm not sure that a DVI-D DL will be able to drive 4k at 60 hz just because it has an adapter, all that data still has to go out the DVI port before it's adapted. the card should also have HDMI 2.0 which should be able to support that bandwidth, but if the monitor doesn't... maybe if there is a HDMI 2.0 to displayport adapter that supports 4096x2160 that could work... let me see what I can find.
     
    Edit: the specs for the ports on that card aren't accurate at Amazon, if you check out the specs on the EVGA website, it shows 1 DVI-I DL, 1 HDMI 2.0 and 3 Displayports, so you should be able to use two of the display ports for that no problem.

    I re-checked the card images on Amazon, they show 3 displayport ports. 
     
    So it can handle two simultaneous screens at 4096x2160? 
     
    I'm thinking that it won't be the best performance for gaming even with one of those screens, but I have no idea. I have not purchased a vide card for a PC since about 2009.

    If you game on a single screen, you should be fine with lowered settings, as mentioned above. You won't need to disconnect the second screen though, as you would have to set it up to run two simultaneously. It will just use your primary monitor for games.
