I just got a replacement LG 31MU97 monitor from LG (old one had too much backlight bleed).
It was working great, but the order of monitors was not how I wanted it.. my main LP 3065, my InFocus projector to the right and the LG last at far right. I wanted the LG in the middle, so I dragged the icon and arranged it the way I wanted. A requester popped up asking if I want to keep the changes, which I clicked Yes. All displays blanked for a second and when they came back on, the LG display was complaining that it was only running at 3840x2160 and had two black bars down the sides. I went to change it, but the highest available resolution was only 3840 under the custom menu in the nVidia driver.
 
I created a custom 4096 resolution, which gave back the full resolution, but now the refresh is 50Hz and Premiere CS6 can no longer display video without dropping over 100 frames every ten seconds. It's stuttering badly and can't play video in a meaningful way now. Before I swapped the monitor order, I was playing 4K video with no dropped frames. Now the system cannot even play 1080 video without dropping 40% of the frames. If I turn off the monitor, the stuttering goes away. I rebooted and I still have the problem.
 
I tried to create a custom 60Hz setting, but the system won't add it to the list of custom resolutions now.
 
What the heck did I break when I rearranged the monitor order??

    When I rebooted the system, the 4K monitor on Displayport reverted to 1080P, showing the desktop in a small rectangle in the center of the monitor.
     
    Apparently there are no resolutions 'available' for that monitor that are higher than 1920x1080, under "Ultra HD, HD SD" menu in nVidia Control Panel. It discarded my custom resolution setting when the system rebooted. This is very puzzling. I had it working well until I replaced the 4K monitor with a new one. nVidia Control Panel properly identifies the monitor as LG Electronics... but beyond that, I don't know why my GTX 680 refuses to default to 4K on that monitor. Maybe I'll try the HDMI connection and see if I get less frame dropping in Premiere.

    HDMI is even worse.. it reverts to 1024x768. Even if I force 4096x2160 @ 24Hz, it reverts. The only way to get it to stay in 4K is to set it to 16-bit color instead of 32-bit.
     
    This graphics driver must have a timing bug when the extended desktop is arranged in certain orders.

    Uninstalling and reinstalling the driver seems to straighten things out. But I must not touch any of the monitor sequence settings. It's still 50Hz, but at least it's 4096x2160 and video playback is smooth. Still, it's baffling how just rearranging the monitor ordering can cause havoc such that Premiere cannot even play HD video without dropping 40% of the frames and that the 4K resolutions disappear from the list of available Ultra-HD, HD and SD resolutions list.
