Looking for recommendations for HTPC

I currently have an HTPC with a Sapphire IPC E-350M1 for the motherboard and it is beginning to act flaky.
 
I was wondering if anyone could save me hours of research and point me in the direction of a suitable replacement?  This HTPC is mostly used for Netflix and Hulu, a bit of video/music from my Synology NAS, and Redbox DVDs/Blu-Rays.
 
I dont mind a whole replacement, but I'll list the rest of the components of the HTPC in case some can be reused;
  • Case: APEX MI-008 ITX case
  • PSU: 20Pin PicoPSU (Will need to look up Watts when I get home, I think 120W)
  • RAM: 4GB (2x2GB SO-DIMM PC3)
  • SDD: 120GB Crucial M4
  • HDD: 750GB Seagate 7200.10
  • ODD: LG Blu-Ray ROM
  • I have a 3.5" USB multi card reader, but have never used it, so I'm tempted to rip it out.  This is only noteworthy if the case is replaced.

    I was looking at this setup for a HTPC/Gamer but gave up, but be fine for HTPC-Only

    Knight - that looks excellent!  Would you recommend the embedded or should I go with the AM1 board and buy a proc (I'm thinking the Athlon 5150)?
     
    Anyone else have a suggestion or is this a solid recommendation?

    Those AMD A10 processors are a pretty solid choice. Also the new Intel i3's with the HD4600 IGP is a great option too. Both of those options are around $130. EVGA is currently doing a deal on their Z87 ITX boards for $79.

    What's ur budget, and are you going to use a psu or DC-brick?

    I'm guessing this is 1080p TV? I'm unsure if the Intel or APU(amd) would do better for playback...

    My budget is pretty flexible, but I want to stay under $150 if possible.  I'm still interested in reusing that PicoPSU, but will transition to a DC brick if necessary.
    The current TV is 720, but I will be upgrading in the near future to 1080 since it is dying.
