4 Months ago I bought the EVGA GeForce GTX 750Ti Superclock w/G-SYNC Support 2GB GDDR5 128bit, Dual-Link DVI-I, HDMI, DP 1.2 Graphics Card (02G-P4-3753-KR) for my PC I built over the summer. It was working fine for those 4 months until recently the video card began to crash. For instance, less than 10 min in after logging in and browsing the internet, my screen would all of a sudden turn either yellow, red, or black. I can't even play games without the screen crashing.
Steps I've taken to try to resolve the problem:
Re-installing the software.
Air dusting the HDMI Cable and re-plugging it back in
Removing the actual card from the slot, air dusting it and putting it back in
Air dusting entire PC
So far none of these things have worked and my video card continues to crash. Right now I switched back to the integrated graphics for now but I wish to find a solution so I'm turning to the community for solutions.
Here are the specs/parts for my PC:

Intel Core i5
Motherboard: Gigabyte GA-H81.Amp-UP ATX LGA1150 Motherboard
Memory: Team Elite 8GB (1 x 8GB) DDR3-1600 Memory
Storage: Seagate Barracuda 500GB 3.5" 7200RPM Internal Hard Drive

What power supply are you using?
Which drivers are you currently using?
Have you monitored the temperatures of your card?

isaac.kuhn do you have precision x controlling the gpu fan? if you don't have it download it from evga.here is a very good guide to help you set it up.

Sounds like the card is defective. You could try the following to see if it helps resolve your issue...
#1 Perform a
clean boot
of windows.
#2 Perform a clean installation of video drivers.
Uninstall Precision X/Afterburner if installed, make sure to select "no" to saving profiles if asked.
Reboot the machine.
Download & save graphics driver
347.25
to your desktop. Run the graphics installer located on your desktop once the download is complete.
Select custom advanced install, under custom installation options uncheck all options except the physx system software, if you are using audio over HDMI please select the HD Audio driver, if you have a 3D capable monitor and would like to use 3D at some point select the 3D Vision driver also, check mark perform clean install, then click next to install the driver, reboot when asked.
#3 Test your machine for awhile to see if the crashing is resolved.
If the crashing is not resolved try the following to see if it helps resolve the crashing...
#1 Download
MSI Afterburner
and set the fan speed (%) to max. Fan should be loud now.
#2 Test your machine for awhile to see if the crashing is resolved.
