A defective monitor will not cause a blue screen. Looks like your card bit the dust. RMA time.

Yes, you can still RMA. Contact EVGA to setup the RMA. http://www.evga.com/about/contactus/

No problem. A reference GTX 980 would go good with your system.

If you must go with a 970 then I would recommend this one because of the case you're using. ACX cards need good case ventilation due to venting hot air into the case. A blower style card would work best for your setup. A blower style card will blow all the hot air out the backside of your case keeping the hot air from being vented into the case.

Your GTX 580 has clearly failed and is exhibiting the classic tell-tale signs of solder cracking somewhere on the PCB..Do not despair, you might be able to easily fix it if you want to..If you don't have the bones to buy another card right now or if you just want to have it as a backup, try fixing it as described below..Up to you about which card to buy should you choose to now, a GTX 970 would be a good choice..My opinion is that if you're going to run higher resolutions than 1080p, to go with a 3GB or 4GB VRAM card..And like Sajin said, you need good case ventilation with an ACX cooler..Prefer blower-style coolers myself.

Disassemble the card down to the bare PCB and bake only the PCB like this..If solder cracking somewhere on the PCB is the problem (almost guarantee you it is), baking it should reflow any cracks that might exist and all will be well, at least for a while, or a long time, nobody can say..This has happened to me on a few different cards and baking fixed them..But with a 9800 GTX that I have, baking would only fix it for a few months or so and then it would start having issues again..So I would bake it again and fix it..I think I ended up baking the stupid thing 4 or 5 times within a year or so after it went bad and each time fixed it, for a while ..It was in use for two years before it failed..I still have it and it works fine..If you bake it, do it only with the GPU side up..There's disassembly guides if you need, here's one that's good enough, there's more if you search around..Alot of ppl have fixed cards by baking, aka "the oven trick".

Hmm I would avoid the GTX 970 if you plan on using more than 3.5GB VRAM:
    http://www.techpowerup.com/209339/gtx-970-memory-drama-plot-thickens-nvidia-has-to-revise-specs.html

It seems that NVIDIA was dishonest to their customers and this is not the first time.

Personally I'd go with a R9 290X 4GB, a much better bang for buck well from where I live, in most cases it beats the GTX 970 with ease and it's about 8% Slower than a GTX 980, but faster that the 980 if AMD Mantle API is used But hey it depends on what you play, a GTX 780 Ti would be  a very good alternative if you plan on sticking with NVIDIA

    Cut out from that article:

"NVIDIA partitioned the 4 GB of memory to two segments. The first is 3.5 GB large, and is made up of memory controllers with access to their own segments of the L2; the second segment is 512 MB in size, and is tied to that memory controller which is rerouted.

The way this partitioning works, is that the 3.5 GB partition can't be read while the 512 MB one is being read. Only to an app that's actively using the entire 4 GB of memory, there will be a drop in performance, because the two segments aren't being read at the same time. The GPU is either addressing the 3.5 GB segment, or the 512 MB one. Hence, there's a drop in performance to be expected, again, for apps that use up the entire 4 GB of memory.

While it's technically correct that the GTX 970 has a 256-bit wide memory interface, and given its 7.00 GHz (GDDR5-effective) memory clock, that translates to 224 GB/s of bandwidth on paper, not all of that memory is uniformly fast. You have 3.5 GB of it having normal access to the crossbar (the town-square of the GPU), and 512 MB of it having slower access. Therefore, the 3.5 GB segment really just has 196 GB/s of memory bandwidth (7.00 GHz x 7 ways to reach the crossbar x 32-bit width per chip), which can be said with certainty. Nor can we say how this segment affects the performance of the memory controller whose crossbar port it's using, if the card is using its full 4 GB. We can't tell how fast the 512 MB second segment really is. But it's impossible for the second segment to make up 28 GB/s (of the 224 GB/s), since NVIDIA itself claims this segment is running slower. Therefore NVIDIA's claims of GTX 970 memory bandwidth being 224 GB/s at reference clocks is inaccurate."

But if you stay within the 3.5GB VRAM buffer you will be fine, I just wanted to pass you the truth that it all it comes from a reliable sources and this news is now everywhere, it is more than true that NVIDIA has again been dishonest towards their customers, just so you know what you are buying a VGA card with 3.5GB VRAM and not 4GB.

No idea on the caps. Sounds like you baked it GPU side down, you were suppose to bake it GPU side up. 13mins was too long also, you were suppose to bake it for 10 mins.
