GTX 680 Event ID 4101, 14

Hello, I am having issues with my Graphics card.  Recently it has been freezing my whole pc including mouse every few seconds then going full black screen across all 3 monitors then returning unfrozen with the error
                     "Display driver nvlddmkm stopped responding and has successfully recovered." (Event ID 4101, in Event Viewer). 
 
I have also had another error 
                    "The description for Event ID 14 from source nvlddmkm cannot be found. Either the component that raises this event is not installed on your local computer or the installation is corrupted. You can install or repair the component on the local computer.

If the event originated on another computer, the display information had to be saved with the event.

The following information was included with the event:

\Device\000000a8
CMDre 00000003 00000080 00000000 00000005 0000000d"  (Event ID 14, in Event Viewer)


I have even gotten a blue screen memory dump.  I know it isn't my hard drives and have run out of ideas.  I have tried everything I can think of to clear this issue including a clean install of my gpu with the newest driver and installing an older driver.  I have never OC'd my gpu and have had no issues until recently. I also had an error with NvidiaStream had no idea what it was I don't own a shield and turned it off.  It seems to always occur when I am watching Twitch or on Skype.  I am out of ideas and was hoping for some help.  Thanks!

    Hello, try the following to see it helps resolve your issue...
     
    #1 Perform a clean boot of windows.
     
    #2 Perform a clean installation of video drivers.
    1. Uninstall Precision X/Afterburner if installed, make sure to select "no" to saving profiles if asked. Reboot the machine.
    2. Download & save graphics driver 347.25 to your desktop. Run the graphics installer located on your desktop once the download is complete. 
    3. Select custom advanced install, under custom installation options uncheck all options except the physx system software, if you are using audio over HDMI please select the HD Audio driver, if you have a 3D capable monitor and would like to use 3D at some point select the 3D Vision driver also, check mark perform clean install, then click next to install the driver, reboot when asked.
    #3 Test your machine for awhile to see if the crashing is resolved.
     
     
    If the crashing is not resolved try the following to see if it helps resolve the crashing...
     
    #1 Download MSI Afterburner and set the fan speed (%) to max. Fans should be loud now.
    #2 Test your machine for awhile to see if the crashing is resolved.

    No problem. Upload your memory dump file too so myself and others can review it, this will help us troubleshoot the issue further if needed.

    Sorry for getting back so late. Hecktic week.  So I did a clean install last night and it crashed once again.  I keep getting Event ID 4101 and 14, I am going to download the MSI Afterburner right now and hope for the best.  If this doesn't work would you suggest going RMA route?

    There are a few other things to checkout before I suggest RMA'ing the card. Report back with your msi afterburner results and we'll go from there.

     Okay so I had tried it for less than 5 minutes and my computer froze again.  It's not even recovering anymore I have to do a hard restart on my computer it just lags constantly.  I have given it 5 mins to try and recover but it remains laggy and frozen.

    What power supply are you using? Does downclocking the video card -105 from default clocks on both the core and memory help resolve the issue? You can downclock the card using afterburner.

    What power supply are you using? Does downclocking the video card -105 from default clocks on both the core and memory help resolve the issue? You can downclock the card using afterburner.


     
    The power supply I am using is the Corsair AX 850 Gold Series.  And I have never overclocked or underclocked and have no idea how to do so.

    Ok, would it possible for you to test the card in another machine to verify the card is really the issue? If not, you should just start the RMA process and hope for the best.

    No, unfortunately this is my only desktop,  Thank you so much for your help  Sajin I will start the RMA right now.  Thankfully EVGA is an amazing company!
