This is surely the result of using crappy old engine design.

Exactly my thought, how much did they try to save vs. lost?? Why cheap out on what is supposed to launch you into space??

Not a crappy design, infact one of the best designed rocket engines ever. However they are 40+ years old, not the best thing to do with rocket engines(yay semantics). Although Orbital can now buy the RD-180 which they're going to use on a new rocket I believe. That or as an interim until they develop a brand new engine and launch vehicle (antares MK2)

Metallurgy wasn't up to snuff in the 60's, so even though you might claim that it's one of the best designed, that does not mean that it was one of the best manufactured. Design and manufacturing are two different ballgames. Do you happen to know what NDT was done to certify them for flight?
