This guy is a Godsend.
I do not know him and this is not a joke.
I have been dealing with Nvidia level 2 and Samsung level 2 and 3 support for well over a month now,
because my 4k Samsung was not working in 4k. It would blackscreen on me when I went into any 4k res.
I have spent hours and hours and hours dealing with this.
I have pulled my hair out and bled inside this computer over this..
After going through owning my gtx 680s and getting no solution then upgrading to the 9 series and still getting no help
At the end of my rope. When I had finally lost all hope and given up.

Anyway,
I got an email from EVGA so I decided to call them up.
We start running through the basics, try one card at a time etc. 
Then Erin W tells me that maybe I need to enable UHD colors..
UHD colors? huh? O.o?!
NEVER,
not even ONCE,
from dealing with all these people who call themselves "professionals" in tech support, did ONE single person even hint at such a thing.
My problem is solved and EVGA has a lifelong buyer now. My mobos, my gpus, hell, if I need a blood transfusion, I'm getting it from EVGA!

solution to new problem added:
I found the solution to the input loss of signal problem, after changing HDMI inputs.
I'd watch TV on HDMI 1, then switch back to my PC on HDMI 2 @4k and would get no signal.
 
Amazing how much Samsung kept trying to tell me it was the GPU.
I had to literally tell three people that they were not qualified to help me.
 
Anyway one of the guys in like level 3 support? Iunno.
I had to leave a call back number for this escalated guy to get in touch with me.
He was completely clueless and admitted it. 
 
He just blindly threw it out in the air to try resetting factory defaults and it worked.
He was just as surprised as I was.. lol
 
From the sound of it, it is a good idea to reset factory defaults on any samsung tv, before you start playing with it.
and definitely after a firmware update.
 
It will not revert to an older firmware.
It will just be like resetting cmos for certain new hardware.
It's a good idea and absolutely necessary, to access some of the TV's settings and fix some problems.
 
It may take up to 30 seconds with the "no input" message on the screen, before the screen actually comes on, when you switch HDMI ports.
So wait for it.
 
EDIT: it stopped working again in random cases.

    Lol. I felt the same way when I spent a week trying to figure out my motherboard issues on my first z77 FTW. Sure its a different story, but I was on the phone for 4 hours, at 3am in the MORNING, and some poor sap named Matt was on the other line, (bashing his head I'm sure) helping me resolve my issues. And they did :) I've never seen a company this great!

    AH, let me guess you where going at 60hz over HDMI? yeah that can only work with a different type of color setting, only display port 1.2 will work at 3840x2160 @ 60 hz with standard colors
    Was an interesting trick Nvidia did to make it work, going from a uh, I think it was a 4:4:4 color or 4:2:0 color to lower bandwidth to the point it would work on current HDMI. Doesn't really effect things like Movies and video (as the source uses 4:2:0 color) but might on things like games. I'm using display port on mine atm for full color.
     
    As much as I've gone off on the subject, glad you where able to be helped out well :)

    Actually I am using Real HDMI 2.0. now.
    At first I was using the Nvidia chromatic subthingamajig though, on my gtx 680's, due to only hdmi 1.4 and DP 1.2 support and my TV only having hdmi 2.0 ports on it.
    On my 980's though, true hdmi 2.0 is supported so it's all legit now. Real UHD, 4k.
     
    I had the same problem even after upgrading to the 9 series though, which I shouldn't have.
    I shouldn't of had the problem with the 680's either.
    Just no one ever told me to "enable  UHD colors" in the Samsung TV settings,  under "picture settings".
     
    Not even after over a month of Samsung and Nvidia "tech support" lol..

    Having a similar issue with a new Vizio P-series, 55" running a GTX970 card.   Nvidia control panel will not even give me the option for a 4K setting at 60Hz.   30Hz, going downward to 23hz.     Doign a customr esolution and tryign to force a 60hz give me a black screen.  
     
      Vizio has requested screenshots of the Nvidia control panel, Dxdiag text files, etc... 
     
      HDMI cable is a Rocketfish RF-HG08501 - 18Gbps, 4K/2k, up to 240Hz according to the packaging.

      Yes, I'm aware of that.    4096x2160 is limited to a 24Hz refresh only.  Between rez and 1280x800, there are no resolution possibilities.  1280x800 will refresh @ 60hz. 
      The highest rez setting that will give 60Hz is the 1920x180 listed under UHD,HD, SD setttings.

    Dam.. 
    I have been noticing that when I switch HDMI ports, even now.
    Sometimes it just doesn't work.
    Even after the fixes.
    Samsung needs a firmware update, big time.
    I expect better out of a company, that acts like it is the best at what it does.
     
    A lot of these 4k sellers blatantly mislead people. Watch out.
    I have heard a lot of people telling me they got stuck with a 24 hz TV. I'd return the TV asap.
    Their tech support will run you though circles, until your time to return the item runs out.
    You should absolutely see the 4k res's.
    Even if they don't work, they should be listed.
    I think you got one of those TV's possibly.

    My HU8550 does 60 Hz/4K on all ports, but the color output is limited to 4:2:0 even though I'm using a GTX 980.
     
    I'd advise not buying a 4K Samsung until they fix it, unless you are fine with 4:2:0 output (personally I'm not as I planned on using this is a secondary monitor and text quality looks terrible). Ongoing thread over at the Nvidia forums on this here:

    Mine is an older model and the text looks perfect.
    I'd consider that it may need a firmware update.
     
    I saw your post and I see you have tried most things..
    What I'd of said to try would be.
    Try HDMI 1 or 2.
    My HDMI 3 won't work.
    Try DVI or DP.
    Verify firmware version.
    Verify windows display settings, say the same thing that nvidia control panel says, for refresh rate and res.
    Mine was different for some reason.
    Had me at 59 hz in windows and 60 in nvidia.
    I changed it to 60 in both.
    Try a reset factory defaults.
    Default pin number should be 0000.
    Make sure you have no hdmi cables plugged in, when you enable UHD color on those ports.
     
    I bet you tried all, if not most of those..
     
    If you get it fixed, please do let us know. 
    I know how sincerely aggravating it is to deal with the "professionals" and then realize most of them, have absolutely no idea what so ever.
    The run around is the pits. Good luck man.
     
    My blackscreen issue is also back when I switch HDMI ports.
    Called Samsung about that and they have someone calling me back.
