EVGA GTX 660 Driver Keeps Crashing Upon Windows Login

Hi all!
 
So I've been having a bunch of issues with my GTX 660. I used to have a 560 which died a few months ago and luckily I was able to RMA it. EVGA sent me a brand new 660 and I was pretty happy. However I've been fighting with the card ever since. Upon logging in, there is a 50% chance of the driver crashing directly after entering my password and hitting enter (pixelated screen, washed out colors, etc). Now if I do make it to the desktop, opening any application (ranging from Chrome, Spotify, etc) will cause the driver to crash (The display driver stopped responding and has recovered error) again leading to the screen flashing between black and the window(s) that were just opened before finally locking up after a few seconds.
 
Here's what I've tried-
1. Completely clean install of new 340 drivers using driver cleaner beforehand to remove any traces of the old drivers.
2. Rollback to the old 337 and 335 drivers
3. Trying it in the other PCI-E slot
4. Updated all motherboard drivers and BIOS to most recent version
5. New, fresh install of Windows
 
Now #5 worked for about two days before the issue appeared again. I was playing some games (World of Tanks) and my screen locked up and subsequently restarted. Ever since that original crash, the same issue has popped up. I've run out of ideas and need some help!

G.SKILL Ripjaws Series 4GB (2 x 2GB) 240-Pin DDR3 SDRAM DDR3 1333
CPU:
Intel Core i5-2500K
500 W PSU
Win 7, 64-bit Enterprise

    Looks like you received a defective card. Have you tried downclocking the video card -105 from default clocks using evga precision or msi afterburner? If not, give it a shot to see if it helps resolve the issue.

    Well, I guess my luck isn't that good haha. I've downclocked it and I'll let you know if it remains stable. Thanks!

    Hi all, just a quick update in case anyone else has a problem like mine. Looks like there might be something wrong with the card as underclocking it by 60 MHz fixed the issue but raising it any further than that still caused some problems. Thanks for all your help!
