So anyways, I am probably the only EVGA card owner and Precision X user that this has ever happened to - losing the icon for the PrecisionX.exe file.
After much trial, I finally managed to get it back.
Apparently my problem lay with the fact that:
I had installed OC Scanner first, which installed to an EVGA folder, then a subfolder for OC Scanner, on my programfilesx86 directory.
When I installed Precision X, it actually created another EVGA folder within the same directory just for Precision X. Not sure how that happened.
So this morning , I uninstalled and reinstalled Precision X, and rather than letting it install to the default directory, I browsed to the EVGA folder that OC Scanner resides in, and let it install into the same folder. So now I have an EVGA folder, with two folders within, one for OC Scanner, the other for Precision X.
And, for whatever reason, this solved the missing icon for the precisionx.exe file.
 
I doubt anyone else will manage to run across this, but thought I would post how it got resolved for me just in case.

Same problem with me, I think it happens when you uninstall the program then reinstall it, just wondering if there"s something else that needs to get deleted. Already tried deleting EVGA folder before reinstall, most likely I think something else needs to get deleted, but can't find it.

i also have this problem is dumb and annoying i notice this ****e happen when you delete the desktop icon it will also **** up the icon under start menu. this needs to be resolve minor problem but annoying!!!!

 
 
I had a similar problem and Someone here posted this fix and it worked for me.  I am very sorry to say that at this minute I cannot remember who posted this fix.  I will try to find out who later when I have some more time.
It may take a few tries to get it to work correctly.

Icon fix
Proceed to an elevated command prompt and ensure you have the administrative privileges. You can do this by doing a Start->Run->Type 'cmd' and then right-click and choose 'run as administrator' to gain this elevated command prompt.

From there you'll want to perform these steps in sequence. This will essentially end the explorer task, change your directory to the local data store in the current user profile, eliminate the IconCache.db file and then restart your PC. Afterwords it will rebuild your icon cache and you may need to set the icons again but they should not revert to the generic IE icon any further:

• taskkill /IM explorer.exe /F
• CD /d %userprofile%\AppData\Local
• DEL IconCache.db /a
• shutdown /r

Another thing you can do is change to a different ICON and the change back to the X one.

Delete your short cuts and remove from your upper start menu and task menu.
Then to the start menu All Programs EVGA and then Precision X and re-pin to your start menu and to your task menu.
This is also if you are on Windows 7 and Below, for Windows 8.x you will need a Windows 8.x user to chime in.
